医联体内患者就医机构选择行为研究

摘　　要：目的:分析医联体内患者选择基层或非基层医疗机构的行为及其影响因素,为促进基层首诊提供政策建议。方法:采用随机抽样方法抽取医联体内医疗机构门诊患者进行问卷调查,应用单因素分析和二元Logistic回归分析,探讨患者就医机构选择的影响因素。结果:“距离近”是患者选择就医机构的首要因素,选择非基层医疗机构就医的患者还关注医疗水平;患者个体特征因素(年龄、学历、职业等)、医疗服务感知因素(取药等候时间、诊疗费用及诊疗结果等)、医联体相关政策认知因素都会影响患者就医机构的选择。结论:应完善医联体内部制度建设,优化外部政策环境,合理引导患者的就医机构选择。Objective To analyze patients’ choice behavior of primary care institutions or not and its influencing factors in regional medical association, and provide policy suggestions for promoting first-visit at primary care institutions. Methods The random sampling method was used to select outpatients from regional medical associations, and questionnaires were delivered. Univariate analysis and binary logistic regression analysis were adopted to explore the influencing factors of patients’ choice of preferred medical institutions. Results Close access to medical services is the primary factor of patients’ choice behavior. Patients who did not choose primary care institutions often pay attention to medical service quality. Influencing factors include Individual characteristics(e.g., age, education, occupation), perceived medical services(e.g., waiting time for taking medicines, costs, and diagnosis and treatment results), and perception of relevant policies in the regional medical associations. Conclusion It is recommended to improve the internal system construction of the regional medical association, optimize the external policy environment, and guide patients’ choice behavior of preferred medical institutions rationally.
