摘  要:目的通过分析广东中山地区不同季节不孕门诊男性患者精子受精功能的变化,为自然受孕及辅助生殖治疗的最佳受精时机提供参考。方法选取2019年3月至2021年5月中山市人民医院不孕门诊采集的精液标本7109份作为研究对象,其中春季(3月至5月)2772份,夏季(6月至8月)1885份,秋季(9月至11月)1579份,冬季(12月至次年2月)873份。对所有精液标本进行精液常规分析,其中414份标本进行精子顶体酶活性定量测定,275份进行诱发精子顶体反应测定,449份进行精子-透明质酸结合测定。结果前向运动(PR)精子数量夏季[(86.8±79.4)×10^(6)个]明显少于春季[(95.11±78.9)×10^(6)个]、秋季[(94.4±81.7)×10^(6)个]、冬季[(100.9±81.5)×10^(6)个],差异具有统计学意义(P<0.05),而秋、冬、春季之间差异无统计学意义(P>0.05);精子顶体酶定量测定值春季[(79.7±30.4)μIU/10^(6)精子],高于夏季[(74.1±34.7)μIU/10^(6)精子]、秋季[(72.4±33.9)μIU/10^(6)精子]、冬季[(71.5±33.1)μIU/10^(6)精子],差异具有统计学意义(P<0.05),而夏、秋、冬季之间差异无统计学意义(P>0.05);诱发精子顶体反应测定值春季[(44.0±14.1)%]和冬季[(46.8±17.2)%]高于夏季[(38.1±11.6)%]和秋季[(41.5±8.7)%],差异具有统计学意义(P<0.05),而春季和冬季及夏季和秋季之间差异无统计学意义(P>0.05);精子-透明质酸结合测定值春季[(57.3±12.3)%]和冬季[(57.4±12.2)%]高于夏季[(49.2±15.3)%]和秋季[(51.8±14.5)%],差异具有统计学意义(P<0.05),而春季和冬季及夏季和秋季之间差异无统计学意义(P>0.05)。结论季节因素对不孕门诊患者精液常规参数及受精功能有影响。整体而言,春、冬季的精子受精功能比夏、秋季要更好,春、冬季是临床受孕的好时机。Objective To provide reference for the best fertilization time of natural conception and assisted reproduction therapy, by analyzing the changes of sperm fertilization function of male infertility clinic patients in different seasons in Zhongshan of Guangdong province. Methods 7,109 semen samples were collected from Infertility Clinic of Zhongshan People′s Hospital from March 2019 to May 2021, including 2,772 semen samples in spring(March to May), 1,885 semen samples in summer(June to August), 1,579 semen samples in autumn(September to November), and 873 semen samples in winter(December to February). Routine semen analysis was performed on all semen samples, including 414 samples for sperm acrosomal enzyme quantitative determination, 275 samples for induced sperm acrosomal reaction determination, and 449 samples for sperm-hyaluronic acid binding determination. Results The number of PR grade sperm in summer [(86.8±79.4)×10^(6)] was significantly lower than that in spring [(95.11±78.9)×10^(6)], autumn [(94.4±81.7)×10^(6)] and winter [(100.9±81.5) ×10^(6)](P<0.05), but there was no statistical difference between autumn, winter and spring(P>0.05). Quantitative measurement of sperm acrosomal enzyme in spring [(79.7±30.4)μIU/10^(6)] was higher than that in summer [(74.1±34.7)μIU/10^(6)], autumn [(72.4±33.9)μIU/10^(6)] and winter [(71.5±33.1)μIU/10^(6)], with statistical difference(P<0.05), but there was no statistical difference between summer, autumn and winter(P>0.05). The sperm acrosomal response induced in spring [(44.0±14.1)%] and winter [(46.8±17.2)%] was higher than that in summer [(38.1±11.6)%] and autumn [(41.5±8.7)%], the differences were statistically significant(P<0.05), but there was no statistical difference between spring and winter and between summer and autumn(P>0.05). Sperm-hyaluronic acid binding assay in spring [(57.3±12.3)%] and winter [(57.4±12.2)%] was higher than that in summer [(49.2±15.3)%] and autumn [(51.8±14.5)%], the differences were statistically significant(P<0.
