摘  要:目的通过检测性别确定区域Y-Box2(SOX2)和高迁移率族蛋白1(HMGB1)在子宫内膜异位症(EMS)患者组织中的表达,探究二者在EMS中表达的相关性及临床意义。方法选取2017年4月至2020年7月湖南省妇幼保健院收治的75例经病理检查确诊为EMS的患者,取其囊肿囊壁组织样本作为异位组;随机选取其中60例患者的在位子宫内膜组织样本作为在位组;另选取同期53例因子宫肌瘤行子宫切除术的患者的正常子宫内膜组织样本作为正常组。采用实时荧光定量聚合酶链反应(PCR)和免疫组化法检测SOX2、HMGB1的表达情况,分析二者与EMS临床病理参数之间的关系;采用Spearman相关性分析SOX2、HMGB1表达的相关性;评估SOX2联合HMGB1对EMS的诊断价值。结果与正常组相比,在位组和异位组组织中SOX2、HMGB1表达水平均显著增加(P<0.05);与在位组相比,异位组中SOX2、HMGB1表达水平均显著增加(P<0.05)。SOX2、HMGB1在异位组患者组织中的表达显著正相关(r=0.613,P=0.000);SOX2和HMGB1阳性表达率与异位组患者临床分期有关(P<0.05);SOX2和HMGB1联合诊断EMS灵敏度为87.80%、特异度为93.48%。结论 EMS患者异位组织中SOX2、HMGB1阳性表达率均显著增加,二者表达水平与EMS患者临床分期有关,检测二者表达水平可能对EMS的病理机制研究具有一定意义。Objective To detect the expression of sex-determining region Y-Box2(SOX2) and high mobility group box-1 protein(HMGB1) in endometriosis(EMS), and explore their correlation and clinical significance in EMS. Methods From April 2017 to July 2020, 75 patients with EMS confirmed by pathological examination in Hunan Maternal and Child Health Care Hospital were selected. The cyst wall tissue samples were taken as the ectopic group, and the eutopic endometrial tissue samples of 60 patients were taken as the eutopic group. The normal endometrial tissue samples from 53 cases of hysteromyoma underwent hysterectomy were selected as normal group. The expression of SOX2 and HMGB1 was detected by real-time fluorescence quantitative PCR and immunohistochemistry, the relationship between them and clinicopathological parameters of EMS was analyzed. The correlation of SOX2 and HMGB1 expression was analyzed by Spearman correlation analysis. The diagnostic value of SOX2 combined with HMGB1 in EMS was evaluated. Results Compared with those in normal group, the expression levels of SOX2 and HMGB1 in eutopic group and ectopic group were significantly higher(P<0.05). Compared with those in eutopic group, the expression levels of SOX2 and HMGB1 in ectopic group were significantly higher(P<0.05). The expression of SOX2 and HMGB1 was positively correlated in EMS(r=0.613, P=0.000). The positive expression rates of SOX2 and HMGB1 were related to the clinical stage of EMS(P<0.05). The sensitivity and specificity of SOX2 combined with HMGB1 in the diagnosis of EMS were 87.80% and 93.48%, respectively. Conclusions The positive expression rates of SOX2 and HMGB1 in the tissues of EMS patients increased significantly, and the expression levels of both are related to the clinical stage of EMS. The detection of the expression levels of both may have certain hints for the study of the pathological mechanism of EMS.
