体外受精-胚胎移植结局中胚胎停育的相关因素分析

摘　　要：目的探讨体外受精-胚胎移植(IVF-ET)结局中胚胎停育的原因。方法选取2020年1月至2021年3月青海省人民医院生殖医学中心收治的396对接受IVF-ET助孕并临床妊娠的不孕夫妇作为研究对象。按妊娠结局是否发生胚胎停育分为对照组(n=340)和胚停组(n=56),对两组的男方年龄、女方年龄、不孕症的类型、精液参数、男性异常染色体核型比例、受精率、平均胚胎数及平均优质胚胎数、新鲜胚胎移植、解冻胚胎移植进行比较,并进行统计学分析。结果胚停组女方平均年龄为(33.29±3.69)岁,大于对照组的(31.59±3.92)岁,差异具有统计学意义(P<0.05)。两组男方年龄、不孕症的类型(原发不孕症和继发不孕症)、精液参数、男性异常染色体核型比例、受精率、平均胚胎数、平均优质胚胎数、新鲜胚胎移植比例、解冻胚胎移植比例比较,差异无统计学意义(P>0.05)。结论在IVF-ET助孕过程中,女方年龄过大是引起胚胎停育的主要原因,因此,女方年龄较大的不孕患者应尽早行IVF-ET助孕,以防发生胚胎停育的不良妊娠结局。Objective To investigate the causes of embryo arrest in vitro fertilization-embryo transfer(IVF-ET) outcome. Methods From January 2020 to March 2021, 396 infertile couples who received IVF-ET assisted pregnancy and were clinically pregnant in Center for Reproductive Medicine of Qinghai People′s Hospital were selected and divided into control group(n=340) and embryo arrest group(n=56) according to the pregnancy outcome. Female age, male age, the type of infertility, semen parameters, male proportion of abnormal chromosome karyotype, fertilization rate, average embryo number and average number of high-quality embryos, fresh embryo transfer, thawed embryo transfer of the two groups were compared and statistical analyzed. Results The average age of the female in the embryo arrest group was(33.29±3.69) years, higher than that in the control group(31.59±3.92) years(P<0.05). There were no significant differences in male age, infertility type(primary infertility and secondary infertility), semen parameters, proportion of male abnormal chromosome karyotype, fertilization rate, average number of embryos, average number of high-quality embryos, fresh embryo transfer and thawed embryo transfer between the two groups(P>0.05). Conclusions In the process of IVF-ET assisted pregnancy, the female age is the main cause of embryo arrest. Therefore, for infertile patients with older female age, IVF-ET assisted pregnancy should be performed as soon as possible to prevent the occurrence of adverse pregnancy outcome of embryo arrest.
