早期通里攻下治疗对危重症患者胃肠功能障碍的防治作用

摘　　要：目的分析早期通里攻下治疗对危重症患者胃肠功能障碍的防治作用。方法随机选取医院重症监护病房(intensive care unit,ICU)2017年12月-2020年12月收治的60例危重症患者,根据不同治疗方案将患者分为研究组、对照组,对照组给予西医常规治疗,研究组在对照组治疗基础上给予早期通里攻下治疗,比较两组治疗效果。结果研究组总有效率(93.33%)显著高于对照组(73.33%),且研究组胃肠功能障碍发生率(6.67%)显著低于对照组(26.67%),差异有统计学意义(χ^(2)=4.3200、4.3200,P<0.05);(2)研究组肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-6(interleukin-6,IL-6)水平均低于对照组,且研究组白细胞介素-10(interleukin-10,IL-10)和胃肠激素PGⅠ、PGⅡ水平高于对照组,G17水平低于对照组,差异有统计学意义(P<0.05)。结论早期通里攻下治疗具有清热解毒、攻积泻下、逐瘀通经、荡涤肠胃的功效,可清除胃肠道毒素及血清炎性介质,提高胃肠激素水平,降低胃肠功能障碍发生率。Objective To analyze the prevention and treatment of gastrointestinal dysfunction in critically ill patients with early Tongli Gongxia therapy.Methods A total of sixty cases of critically ill patients admitted to ICU of our hospital from December 2017 to December 2020 were randomly selected and divided into study group and control group according to different treatment schemes.The control group was given conventional western medicine treatment,and the study group was given early Tongli Gongxiu treatment on the basis of the treatment of the control group,and the treatment effects of the two groups were compared.Results The total effective rate of the study group(93.33%)was significantly higher than that of the control group(73.33%),and the incidence of gastrointestinal dysfunction in the study group(6.67%)was significantly lower than that of the control group(26.67%),the difference was statistically significant(χ^(2)=4.3200,4.3200,P<0.05).(2)The levels of tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)in the study group were lower than those in the control group,and the levels of interleukin-10(IL-10)and gastrointestinal hormones PG Ⅰ、PG Ⅱ in the study group were higher than those in the control group,G17 was lower than the control group,the difference was statistically significant(P<0.05).Conclusion Early Tongli Gongxia treatment has the effect of clearing away heat and detoxification,attacking the accumulation of diarrhea,removing blood stasis and meridians,and cleaning the stomach and intestines.It can remove gastrointestinal toxins and serum inflammatory mediators,improve the level of gastrointestinal hormones and reduce the incidence of gastrointestinal dysfunction.
