生殖器传染性软疣1例并文献复习

摘　　要：患者男,38岁,包皮皮疹1月余,无瘙痒及疼痛不适感。皮肤科检查:包皮群集圆形或椭圆形肤色丘疹,部分融合,质地柔韧而坚硬,表面稍粗糙,中央无脐窝,少许破溃、渗出、结痂,躯干及四肢未见异常皮损。皮肤镜检查示:镜下可见中央无结构多行性模式,周围环绕细长扩张的毛细血管。皮损组织病理示:棘层显著肥厚,向下伸入真皮,表皮细胞质内可见软疣小体。诊断:生殖器传染性软疣。治疗:去除皮损,局部皮损给予CO_(2)激光处理后,镊子夹除。外用莫匹罗星软膏,3次/d;局部皮下给予注射用人干扰素α-1b 30μg,隔日1次;同时用3 mL灭菌注射用水溶解3支注射重组人干扰素α-1b, 2~3次/d外用。术后皮损即刻清除,随访半年未复发。A 38-year-old male presented with a foreskin rash for more than one month without itching and pain.Dermatological examination showed foreskin clusters round or oval complexion pimples, partial fusion, flexible and hard texture, slightly rough surface, central umbilical fossa, a little rupture, oozing, scab, and no abnormal skin lesions on trunk and limbs.Dermoscopy showed an unstructured multi-row pattern in the center, surrounded by elongated and enlarged capillaries.Skin pathological examination showed that the spinous layer was significantly hypertrophy, extending down into the dermis, and molluscum corpuscles can be seen in the epidermal cytoplasm.Diagnosis: genital molluscum contagiosum.After the local skin lesions were treated with CO_(2)laser, they were removed with tweezers.Topical mupirocin ointment was used 3 times a day.Recombinant human interferon α-1 b for injection 30 μg was subcutaneously administered locally, once every other day.At the same time, 3 mL of sterile water for injection was used to dissolve 3 injections of recombinant human interferon α-1 b, 2 to 3 times per day topically.The skin lesions were removed after the surgery, without recurrence during the follow-up for half a year.
