摘　　要：目的探讨血浆半乳糖凝集素3(Gal-3)水平与老年慢性射血分数保留的心力衰竭(HFpEF)患者预后的关系。方法连续收集2017年1月~2019年6月于首都医科大学附属北京安贞医院心内科门诊就诊的老年慢性HFpEF患者230例,依据血浆Gal-3水平的三分位数将患者分为1分位组77例,2分位组77例及3分位组76例。随访1年,终点为全因死亡和心力衰竭再住院,用堪萨斯城心肌病调查问卷(KCCQ)评估生活质量,用Cox比例风险回归模型和logistic回归模型分析相关性。结果 1、2、3分位组Gal-3水平呈明显升高趋势[(6.7±2.2)μg/L vs(15.8±3.1)μg/L vs(38.3±10.3)μg/L,P=0.000]。1、2、3分位组随访1年的全因病死率和心力衰竭再住院率逐渐升高(P;<0.01)。多因素Cox回归模型发现,血浆Gal-3水平是全因死亡和心力衰竭再住院的独立风险预测因子(95%CI:1.020~1.062,95%CI:1.029~1.060,P=0.000)。logistic回归分析显示,血浆Gal-3水平与KCCQ评分明显相关(P=0.017)。结论对于老年HFpEF患者基线血浆Gal-3水平不仅是1年全因死亡或心力衰竭再住院风险的独立预测因子,而且与随访1年时的生活质量密切相关。Objective To study the relationship between serum galectin-3(Gal-3) level and outcomes in elderly HFpEF patients.Methods Two hundred and thirty elderly HFpEF patients admitted to our hospital from January 2017 to June 2019 were divided into low Gal-3 group(n=77),medium Gal-3 group(n=77) and high Gal-3 group(n=76).The patients were followed up for 1 year.The quality of their lives was assessed with KCCQ.The relationship of serum Gal-3 with all-cause death and readmission due to HF was analyzed by Cox proportional harzards and logistic regression analysis respectively.Results The mean serum Gal-3 level was significantly lower in low Gal-3 group than in medium and high Gal-3 groups(6.7±2.2 μg/L vs 15.8±3.1 μg/L vs 38.3±10.3 μg/L,P=0.000).The all-cause mortality and readmission rate due to HF were gradually increased in 3 groups during the 1-year follow-up period(P;<0.01).Multivariate Cox regression analysis displayed that serum Gal-3 level was an independent predictor for all-caue death and readmission due to HF(95%CI:1.020-1.062,95%CI:1.029-1.060,P=0.000).The serum Gal-3 level was closely related with KCCQ(P=0.017).Conclusion Baseline serum Gal-3 level is an independent predictor for all-cause death and readmission due to HF and is closely related with the quality of life in elderly HFpEF patients during the 1-year follow-up period.
