基于文献挖掘的《小儿卫生总微论方》治疗小儿惊痫用药规律探讨

摘　　要：目的本研究探讨《小儿卫生总微论方》中小儿惊痫的处方用药规律,为当代医家辨治本病提供思路。方法统计《小儿卫生总微论方》中小儿惊痫有效方剂的药物运用情况,并整理其方(剂型)与药(功效、性、味及归经),采用Excel 2020软件建立原始资料数据库;运用SPSS 25.0软件对数据库资料做描述性统计;使用Weka 3.8软件挖掘出药物间潜在关联规则及应用熵层次聚类法得出组合新方;利用Cytoscape 3.6. 1软件将药物关联规则制成网络视图。结果《小儿卫生总微论方》收录治疗小儿惊痫的方剂共118首,涉及中药共118味,经统计高频药物为全蝎、朱砂、麝香、胆南星、白附子及牛黄等;其中以息风止痉药居多,其余依次为开窍药、化痰止咳平喘药及重镇安神药;药性多为平、温、寒、凉性;药味多属辛、甘、苦味;归经以肝、心、脾经为多;处方剂型以散、丸剂居多。依据关联规则分析,药对支持度最高为朱砂—全蝎,其余依次为麝香—全蝎、麝香—朱砂、胆南星—全蝎、胆南星—朱砂等;并在此基础上进行熵层次聚类法分析得到新方5首。结论本研究结果发现《小儿卫生总微论方》中治疗小儿惊痫多以清肝息风以镇惊、清心定惊以安神、健脾祛痰以开窍为核心治法,对当代医家在临床上对于小儿惊痫辨证施治用药及开拓诊疗新思路具有一定的指导意义。Objective The purpose of this study is to explore the prescription and medication rules of infantile convulsion in Xiaoer Weisheng Zong Wei Lun Fang(小儿卫生总微论方,general and essential prescription of infantile health),so as to provide ideas to treat this disease.Method Analyze the drug use of effective prescriptions for infantile convulsion in general micro prescription for infantile health,and sort out the prescriptions(dosage forms)and drugs(efficacy,nature,taste and meridian tropism),using Excel 2020 software to establish the original database;using SPSS 25.0 software to do descriptive statistics of the database;using Weka 3.8 software to mine the potential association rules between drugs and apply the entropy hierarchical clustering method to get the new combination formula;using the Cytoscape 3.6.1 software to make the drug association rules into the network view.Results There are 118 prescriptions for the treatment of infantile convulsion and epilepsy,involving 118 kinds of traditional Chinese medicine.According to statistics,the high-frequency drugs are scorpion,cinnabar,musk,dannanxing,baifuzi,Niuhuang,etc;the most common drugs are Xifeng Zhijing,followed by Kaiqiao,Huatan Zhike Pingchuan and Zhongzhen Anshen;the most common drugs are mild,warm,cold and cool;the most common drugs are pungent,sweet and bitter.Most of the prescriptions were powder and pill.According to the analysis of association rules,cinnabar scorpion has the highest drug pair support,followed by musk-scorpion,musk-cinnabar,dannanxing-scorpion,dannanxing-cinnabar and so on;five new prescriptions were obtained by entropy hierarchical clustering analysis base on this results.Conclusion The results of this study indicate that the treatment of infantile convulsion epilepsy in the general micro prescription of infantile health mainly was focused on clearing the liver and calming the wind,calming the nerves,invigorating the spleen and eliminating phlegm,and opening the orifices,which have a certain guideline for contemporary doctors
