中药外治法治疗急性闭合性软组织损伤处方用药规律分析

摘　　要：目的分析中药外治法治疗急性闭合性软组织损伤的处方用药规律。方法采用计算机检索中国知网、万方数据库、维普中文科技期刊数据库中中药外治法治疗急性闭合性软组织损伤的随机临床试验,检索时段为2011年1月至2020年11月,采用Excel2013软件录入及提取数据,采用SPSS 21.0统计学软件对数据进行聚类分析,采用SPSS Modeler 18软件进行高频药物的关联规则分析。结果共纳入处方39个,涉及108味中药,使用频次不低于5次的中药共29味,总频次为404次,包括大黄、黄柏、杜仲、乳香、没药等。方中以活血化瘀药出现的味数及频次最高,分别为9味(31.03%)和88次(21.78%);药物归经数量以归肝经最多,药味以辛、苦为主,药性以温热重于寒凉,使用频次不低于5次的中药可聚为三类,共得到12组具有核心意义的药物组合。结论中药外治法治疗急性闭合性软组织损伤的用药规律符合石氏伤科"以气为主,以血为先"的理论,活气行血为筋伤病的基本治则,同时可联用兼具其他本地特色的药物组成核心药物组合来治疗。Objective To analyze the medication principles of the prescriptions for external therapy of Chinese herb medicines in the treatment of acute closed soft tissue injury.Methods The randomized clinical trials(RCTs) of external therapy of Chinese herb medicines in the treatment of acute closed soft tissue injury in CNKI,WanFang,VIP database were searched from January 2011 to November 2020.The data were entered and extracted by the Excel 2013 software,the cluster analysis of the collected data was conducted by the SPSS 21.0 statistical software,and the association rule analysis of high-frequency Chinese herb medicines was conducted by the SPSS Modeler 18 software.Results A total of 39 prescriptions were included,involving 108 Chinese herb medicines,of which 29 Chinese herb medicines with a frequency≥five times,with a total frequency of 404 times,including Rhei Radix et Rhizoma,Phellodendri Chinensis Cortex,Eucommiae Cortex,Frankincense and Myrrha.Among the prescriptions,the Chinese herb medicines for invigorating blood circulation and eliminating blood stasis had the highest number and frequency,which were nine kinds(31.03%) and eighty-eight times(21.78%),respectively.The number of Chinese herb medicines returning to the liver meridian was the highest,the flavor was mainly pungent and bitter,the nature was warm and hot rather than cold and cool,and the Chinese herb medicines with a frequency≥five times could be grouped into three categories,and a total of 12 groups of related couplet medicinals with core significance were obtained.Conclusion The medication principles of the prescriptions for external therapy of Chinese herb medicines in the treatment of acute closed soft tissue injury is in line with the theory of "Qi first,blood first" of Shi’s Traumatology.Activating Qi and blood is the basic treatment principle of tendon injury.Meanwhile,drug groups composed of Chinese herb medicines with other local characteristics can be used for the treatment of tendon injury.
