摘 要:目的:分析诱导性支气管相关淋巴组织(iBALT)相关细胞因子CXCL13在慢性阻塞性肺疾病急性加重期慢性阻塞性肺疾病患者(AECOPD)肺泡灌洗液(BALF)中的变化及临床意义。方法:选取14例因细菌感染诱发AECOPD的患者为AECOPD组,并选取同期24例急性细菌性肺炎患者为单纯感染组。采用支气管镜采集两组患者BALF,ELISA测定BALF中CXCL13、IL-8、IL-6及TNF-α水平。比较两组患者CXCL13、IL-8、IL-6及TNF-α水平,并分析CXCL13与IL-8、IL-6及TNF-α的相关性。结果:AECOPD组CXCL13(P=0.016)、IL-8(P=0.082)、IL-6(P=0.008)、TNF-α(P=0.201)水平均高于单纯感染组。AECOPD组CXCL13与IL-8(r=-0.925,P<0.001)、TNF-α(r=-0.886,P<0.001)、IL-6(r=-0.525,P=0.054)呈负相关,但与IL-6差异无统计学意义。单纯感染组CXCL13与IL-8(r=-0.789,P<0.001)、TNF-α(r=-0.480,P=0.018)、IL-6(r=-0.289,P=0.171)呈负相关,但与IL-6差异无统计学意义。结论:与急性细菌性肺炎患者相比,AECOPD患者iBALT面积显著增加,有利于启动机体抗感染免疫反应。Objective:To analyze change and significance of inducible bronchus-associated lymphoid tissue(i BALT)related cytokines CXCL13 in bronchoalveolar lavage fluid(BALF)of patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods:14 patients with AECOPD caused by bacterial infection were selected as AECOPD group,and 24 patients with acute bacterial pneumonia in same period were selected as infection group. BALF fluid of two groups was collected by bronchoscope,and levels of CXCL13,IL-8,IL-6 and TNF-α in BALF were detected by ELISA. CXCL13,IL-8,IL-6 and TNF-α levels were compared between two groups,and correlation between CXCL13 and IL-8,IL-6,TNF-α were analyzed.Results:CXCL13(P=0.016),IL-8(P=0.082),IL-6(P=0.008)and TNF-α(P=0.201)levels in AECOPD group were higher than those in infection group.In AECOPD group. CXCL13 were negative correlated with IL-8(r=-0.925,P<0.001),TNF-α(r=-0.886,P<0.001)and IL-6(r=-0.525,P=0.054),but has ho statistical significance with IL-6. In infection group,CXCL13 were negative correlated with IL-8(r=-0.789,P<0.001),and TNF-α(r=-0.480,P=0.018)and IL-6(r=-0.289,P=0.171),but has no statistical significance with IL-6.Conclusion:Compared with patients with acute bacterial pneumonia,area of i BALT was increased in patients with AECOPD,which was helpful to initiate anti-infective immune response.
