摘  要:目的探讨振幅整合脑电图(aEEG)、脐血pH及新生儿Apgar评分对新生儿窒息结局的预测价值。方法回顾性分析2018年6月至2019年6月在东莞市人民医院儿科接受治疗的50例足月低Apgar评分新生儿的临床资料。比较不同aEEG异常分度新生儿的Apgar评分,不同Apgar评分新生儿aEEG的异常率和不同aEEG异常程度新生儿的脐动脉血气PH和剩余碱的差异。结果轻度窒息新生儿的aEEG状况明显优于重度窒息新生儿,差异有统计学意义(P<0.05);aEEG正常组新生儿的1 min Apgar评分、5 min Apgar评分和10 min Apgar评分明显高于轻度异常组和重度异常组,且轻度异常组明显高于重度异常组,差异均有统计学意义(P<0.05);1 min Apgar评分新生儿的aEEG异常率明显低于5 min和10 min Apgar评分新生儿的aEEG异常率,且5 min Apgar评分新生儿的aEEG异常率明显低于10 min Apgar评分新生儿的aEEG异常率,差异均有统计学意义(P<0.05);aEEG正常组新生儿的pH值明显高于轻度异常组和重度异常组,且轻度异常组又明显高于重度异常组,差异均有统计学意义(P<0.05);aEEG正常组新生儿的剩余碱浓度明显高于轻度异常组和重度异常组,且轻度异常组又明显高于重度异常组,差异均有统计学意义(P<0.05)。结论aEEG、脐血pH及新生儿Apgar评分对新生儿窒息结局具有较高的预测价值,可作为早期诊断新生儿窒息的依据之一。Objective To analyze the predictive value of amplitude integrated electroencephalogram(aEEG),cord blood pH and neonatal Apgar score for neonatal asphyxia outcomes.Methods A retrospective analysis was conducted on the clinical data of 50 full-term newborns with low Apgar scores who were treated in the Department of Pediatrics at Dongguan People's Hospital from June 2018 to June 2019.The Apgar scores were compared between newborns with different aEEG abnormal degrees,aEEG abnormal rates were compared in newborns with different Apgar score,and the differences in umbilical artery blood gas pH and residual base were compared between newborns with different aEEG abnormal degree.Results The aEEG status of newborns with mild asphyxia was significantly better than that of newborns with severe asphyxia(P<0.05).The 1 min Apgar score,5 min Apgar score,and 10 min Apgar score of newborns were significantly higher in the aEEG normal group than in the mildly abnormal group and the severely abnormal group,and the scores in the mildly abnormal group was significantly higher than those in the severely abnormal group;all differences were statistically significant(P<0.05).The aEEG abnormal rate of newborns with 1 min Apgar score was significantly lower than that of newborns with 5 min and 10 min Apgar score,and the aEEG abnormal rate of newborns with 5 min Apgar score was significantly lower than that of newborns with 10 min Apgar score(all P<0.05).The pH value of newborns in the aEEG normal group was significantly higher than that of the mildly abnormal group and the severely abnormal group,and the value in the mildly abnormal group was significantly higher than that in the severely abnormal group(P<0.05).The residual alkali concentration of newborns in the aEEG normal group was significantly higher than that of the mildly abnormal group and the severely abnormal group,and the concentration in the mildly abnormal group was significantly higher than that in the severely abnormal group;all differences were statistically significant(P<
