针药并用治疗脾虚痰湿型慢性分泌性中耳炎的疗效观察及对患者血清SIL-2R、HIF-1α水平的影响

摘　　要：目的观察针药并用治疗脾虚痰湿型慢性分泌性中耳炎的临床疗效。方法将63例脾虚痰湿型慢性分泌性中耳炎患者随机分为对照组31例和观察组32例。对照组予常规药物治疗,观察组在此基础上予针刺治疗。比较两组治疗前后纯音气导听阈、鼓室声导抗图、七项咽鼓管功能障碍症状评分量表(ETDQ-7)评分及血清可溶性白介素2受体(SIL-2R)和缺氧诱导因子1α(HIF-1α)水平变化,并比较两组临床疗效。结果观察组总有效率高于对照组(P<0.05);治疗后,两组0.5 kHz、1 kHz、2 kHz的纯音气导听阈均下降(P<0.05),且观察组低于对照组(P<0.05);治疗后,观察组A型鼓室声导抗图比例高于对照组(P<0.05);治疗后,两组ETDQ-7评分均降低(P<0.05),且观察组低于对照组(P<0.05);治疗后,两组血清SIL-2R、HIF-1α水平均降低(P<0.05),且观察组低于对照组(P<0.05)。结论针药并用治疗脾虚痰湿型慢性分泌性中耳炎的疗效显著,可提高患者听力水平,改善鼓室声导抗图、咽鼓管功能,这可能与其降低血清SIL-2R、HIF-1α水平有关。Objective To observe the clinical efficacy of acupuncture medication combined for chronic secretory otitis media of spleen deficiency and phlegm-dampness type.Method Sixty-three patients with chronic secretory otitis media of spleen deficiency and phlegm-dampness type were randomized to a control group(31 cases)and an observation group(32 cases).The control group received conventional medication and the observation group,acupuncture in addition.Pure tone air conduction thresholds,tympanum acoustic impedance diagrams,the seven-item eustachian tube dysfunction questionnaire(ETDQ-7)scores,and serum soluble interleukin-2 receptor(SIL-2R)and hypoxia-inducible factor-1α(HIF-1α)levels were compared between the two groups before and after treatment.The clinical therapeutic effects were also compared between the two groups.Result The total efficacy rate was higher in the observation group than in the control group(P<0.05).After treatment,the pure tone air conduction thresholds of 0.5 khz,1 khz and 2 khz decreased in the two groups and were lower in the observation group than in the control group(P<0.05).After treatment,the percentage of type A tympanum acoustic impedance diagram was higher in the observation group than in the control group(P<0.05).After treatment,the ETDQ-7 score decreased in the two groups and was lower in the observation group than in the control group(P<0.05).After treatment,serum SIL-2R and HIF-1αlevels decreased in the two groups and were lower in the observation group than in the control group(P<0.05).Conclusion Acupuncture medication combined has a marked effect in treating chronic secretory otitis media of spleen deficiency and phlegm-dampness type.It can raise the hearing level and improve tympanum acoustic impedance diagram and eustachian tube function in the patients,which may be related to its reducing serum SIL-2R and HIF-1αlevels.
