加味黄芪参麦汤对气阴两虚型急性肾功能损伤患者肾功能及炎症反应和氧化应激的影响

摘　　要：目的:探讨加味黄芪参麦汤在急性肾功能损伤患者(气阴两虚型)治疗中的应用价值。方法:所有急性肾功能损伤患者(气阴两虚型)均于2018年5月至2021年4月在本院就诊并随机分两组,对照组63例患者采用西医常规治疗,观察组63例联合加味黄芪参麦汤口服治疗,治疗1周后比较疗效。结果:治疗后观察组血清IL-18、胱抑素C(Cys-C)、丙二醛(MDA)低于对照组,SOD高于对照组,差异具统计学意义(P<0.05);治疗后观察组血清尿素氮(BUN)、肌酐(Scr)、尿IGFBP7低于对照组,24 h尿量高于对照组,差异具统计学意义(P<0.05);治疗后观察组中医总症状积分(主要症状水肿、尿少,次要症状五心烦热、乏力)低于对照组,生活质量评分(QOL评分)高于对照组(P<0.05);观察组总有效率96.83%(61/63)高于对照组的79.37%(50/63),差异具统计学意义(P<0.05)。结论:对于急性肾功能损伤患者(气阴两虚型)而言联合加味黄芪参麦汤有助于减轻炎症反应,调节抗氧化能力,促进肾功能恢复,提高临床疗效。Objective:To explore the application value of modified Huangqi Shenmai decoction in the treatment of acute renal function injury(Qi and Yin deficiency type).Methods:All patients with acute renal function injury(deficiency of both Qi and Yin)were admitted to our hospital from May 2018 to April 2021 and randomly divided into two groups.63 patients in the control group were treated with conventional Western medicine,and 63 patients in the observation group were treated with orally supplemented Huangqi Shenmai decoction.The efficacy was compared after 1 week of treatment.Results:After treatment,the serum levels of IL-18,Cystatin C(Cys-C)and malondialdehyde(MDA)in observation group were lower than those in control group,and SOD was higher than those in control group,with statistical significance(P<0.05).After treatment,the serum urea nitrogen(BUN),creatinine(SCR)and urine IGFBP 7 in observation group were lower than those in control group,and the urine volume at 24 h was higher than that in control group,the difference being statistically significant(P<0.05).After treatment,the total scores of TCM symptoms(main symptoms of edema and oliguria,minor symptoms of five irritability,heat and fatigue)in the observation group were lower than those in control group,and the QOL score was higher than that in control group(P<0.05).The total effective rate of observation group was 96.83%(61/63),which was higher than that of control group(79.37%,50/63)(P<0.05).Conclusion:For patients with acute renal function injury(deficiency of both Qi and Yin),combined with modified Huangqi Shenmai decoction can reduce inflammatory reaction,regu⁃late antioxidant capacity,promote renal function recovery and improve clinical efficacy.
