清热利湿祛脂汤联合米诺地尔治疗湿热蕴结型脂溢性脱发的临床观察

摘　　要：目的探究清热利湿祛脂汤联合米诺地尔在湿热蕴结型脂溢性脱发治疗中的应用价值。方法选取2019年1月—2020年8月期间桂林市中医医院皮肤科收治的94例脂溢性脱发患者作为研究对象,采用随机数字表法分为对照组和治疗组,每组各47例。对照组给予米诺地尔治疗,治疗组在米诺地尔基础上联合清热利湿祛脂汤治疗,治疗期间规律作息,保持心情舒畅,控制脱发诱因。治疗6个月后,观察比较两组患者临床疗效、安全性及治疗前后中医证候积分,心理状态[焦虑自评量表(Self-rating anxiety scale,SAS)、抑郁自评量表(Self-rating depression scale,SDS)],皮肤病生活质量量表(Dermatology life quality index,DLQI)改善情况。结果治疗后治疗组总有效率为93.48%(43/46),对照组总有效率为77.27%(34/44),两组比较,差异有统计学意义(P<0.05)。两组患者治疗后脱发、瘙痒、油腻、鳞屑中医证候积分均较治疗前降低,差异有统计学意义(P<0.05);且治疗组治疗后各项中医证候积分均较对照组明显降低,差异有统计学意义(P<0.05)。两组患者治疗后SAS、SDS评分均较治疗前降低,差异有统计学意义(P<0.05);且治疗组治疗后SAS、SDS评分均较对照组明显降低,差异有统计学意义(P<0.05)。两组患者治疗后DLQI评分均较治疗前降低,差异有统计学意义(P<0.05);且治疗组治疗后DLQI评分较对照组明显降低,差异有统计学意义(P<0.05)。两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论清热利湿祛脂汤联合米诺地尔治疗脂溢性脱发安全、有效,值得临床推广应用。Objective To explore the application value of Qingre Lishi Quzhi decoction combined with minoxidil in the treatment of seborrheic alopecia of damp heat accumulation type.Method From January 2019 to August 2020,94 patients with seborrheic alopecia admitted to the Department of Dermatology,Guilin Hospital of Traditional Chinese Medicine were selected as the research objects.The random number table was used to divide the control group and the treatment group,with 47 cases in each group.The control group was treated with minoxidil,and the treatment group was treated with Qingre Lishi Quzhi Decoction on the basis of minoxidil.Both groups were treated for 6 months.During the treatment,regular work and rest,maintain a good mood,control hair loss incentives.The clinical efficacy,safety and improvement of TCM syndrome score,psychological state[anxiety(SAS),depression(SDS)]and quality of life(DLQI score)before and after treatment were compared between the two groups.Result After treatment,the total effective rate in the treatment group was 93.48%(43/46),and the total effective rate in the control group was 77.27%(34/44).The difference between the two groups was statistically significant(P<0.05).After treatment,the scores of hair loss,itching,greasy,and scaly TCM syndromes of the two groups of patients were lower than before treatment,and the difference was statistically significant(P<0.05);and the scores of various TCM syndromes of the treatment group were significantly lower than those of the control group after treatment,The difference was statistically significant(P<0.05).The SAS and SDS scores of the two groups of patients after treatment were lower than before treatment,and the difference was statistically significant(P<0.05);and the SAS and SDS scores of the treatment group were significantly lower than those of the control group after treatment,and the difference was statistically significant(P<0.05).The DLQI scores of the two groups of patients after treatment were lower than before treatment,and the difference was
