摘 要:目的:探讨25-羟维生素D[25(OH)D]水平与2型糖尿病(T2DM)胰岛素抵抗、叶酸(FA)、同型半胱氨酸(Hcy)的相关性。方法:选择T2DM患者113例作为研究对象,根据25(OH)D水平将所有患者分为三组,25(OH)D低水平组29例为25(OH)D<20μg/L,25(OH)D中水平组50例为25(OH)D水平20~30μg/L,25(OH)D高水平组34例为25(OH)D水平>30μg/L。检测三组胰岛素抵抗指数(HOMA-IR)、FA及Hcy水平,分析25(OH)D水平与HOMA-IR、FA及Hcy水平的相关性。结果:25(OH)D低水平组25(OH)D、FA水平最低,而HOMA-IR及Hcy水平最高,三组组间对比,差异有统计学意义(P<0.05)。经双变量Pearson直线相关检验结果显示,T2DM患者25(OH)D与FA呈正相关(r>0,P<0.05),与HOMA-IR及Hcy水平呈负相关(r<0,P<0.05)。结论:T2DM患者25(OH)D水平与血清FA呈正相关,而与HOMA-IR及血清Hcy呈负相关,临床可加强对上述水平的监测以为临床制定诊疗方案提供重要参考。Objective To explore the correlation between 25-hydroxyvitamin D[25(OH)D]level and type 2 diabetes(T2DM)insulin resistance,folic acid(FA),homocysteine(Hcy).Method 113 patients with T2DM were selected as the research objects.All patients were divided into 3 groups according to their 25(OH)D levels.29 cases in The low level group of 25(OH)D were 25(OH)D<20μg/ml,50 cases in the medium level group of 25(OH)D had 25(OH)D levels of 20~30μg/L,34 cases in the high level group of 25(OH)D had 25(OH)D levels>30μg/L.The three groups of insulin resistance index(HOMA-IR),FA and Hcy levels were detected,and the correlation between 25(OH)D levels and HOMA-IR,FA and Hcy levels was analyzed.Results The 25(OH)D and FA levels in the low level group of 25(OH)D were the lowest,while HOMA-IR and Hcy levels were the highest.There was a statistically significant difference between the three groups(P<0.05).The results of the bivariate Pearson linear correlation test showed that 25(OH)D in T2DM patients was positively correlated with FA(r>0,P<0.05);it was negatively correlated with HOMA-IR and Hcy levels(r<0,P<0.05).Conclusion The 25(OH)D level of T2DM patients is positively correlated with serum FA,and negatively correlated with HOMA-IR and serum Hcy.Clinical monitoring of the above levels can be strengthened to provide important references for clinically designated diagnosis and treatment plans.
