盐酸胺碘酮辅助治疗对小儿心律失常的疗效及甲状腺激素水平的影响

摘　　要：目的探讨盐酸胺碘酮辅助治疗对小儿心律失常的疗效及甲状腺激素水平的影响。方法选取2018年7月至2019年7月本院收治的82例心律失常患儿,随机分为对照组与研究组,每组41例。对照组采用盐酸普罗帕酮片联合酒石酸美托洛尔片治疗,研究组在对照组基础上联合盐酸胺碘酮片治疗,比较两组临床疗效、甲状腺激素水平、24 h动态心电图的变化及不良反应发生情况。结果干预后,两组T_(3)显著低于干预前,T_(4)、TSH显著高于干预前,房性早搏、短阵房速、室性早搏、短阵室速的发作次数均低于干预前(P<0.05);干预后,研究组T_(3)显著低于对照组,T_(4)、TSH显著高于对照组,房性早搏、短阵房速、室性早搏、短阵室速的发作次数均低于对照组(P<0.05)。研究组治疗总有效率显著高于对照组(P<0.05)。两组不良反应发生率比较差异无统计学意义。结论盐酸胺碘酮辅助治疗有助于提高小儿心律失常的治疗效果,能平稳心率,对甲状腺功能影响较小,不良反应较轻,值得临床推广。Objective To investigate the effect of Amiodarone hydrochloride adjuvant therapy on children's arrhythmia and thyroid hormone.Methods 82 children with arrhythmia admitted to our hospital from July 2018 to July 2019 were selected,and they were randomly divided into control group and study group,with 41 cases in each group.The control group received propafenone hydrochloride tablets combined with metoprolol tartrate tablets,the study group received with amiodarone hydrochloride tablets on the basis of the control group.The clinical effect,thyroid hormone level,24-hour Holter changes and adverse reactions were compared between the two groups.Results After the intervention,T_(3) in the two groups were significantly lower than before the intervention,T_(4) and TSH were significantly higher than before the intervention,and the attack times of atrial premature beats,short array atrial tachycardia,ventricular premature beats and short array ventricular tachycardia were lower than those before the intervention(P<0.05);after the intervention,T_(3) in the study group was significantly lower than that in the control group,T_(4) and TSH were significantly higher than those in the control group,and the attack times of atrial premature beat,short array atrial tachycardia,ventricular premature beat and short array ventricular tachycardia in the study group were lower than those in the control group(P<0.05).The total effective rate of the study group was significantly higher than that of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion Amiodarone hydrochloride is effective in the treatment of children's arrhythmia.It can stabilize the heart rate,have less effect on thyroid function and have slight adverse reactions.It is worthy of clinical application.
