右归丸对慢性肾脏病脾肾阳虚证患者矿物质和骨代谢的影响

摘　　要：目的探讨右归丸对慢性肾脏病(CKD)脾肾阳虚证患者矿物质和骨代谢的影响。方法选择2019年6月—2020年7月商丘市立医院肾内科收诊并行规律血液透析的78例老年CKD患者,以随机数字表法分为对照组和研究组,各39例。2组患者均行每周3次血液透析联合每2周1次血液灌流治疗,并予以西药常规治疗,研究组则在此基础上给予右归丸口服治疗,2组均干预3个月,比较干预前后2组患者矿物质和骨代谢指标变化。结果干预3个月后,2组患者Ca、P、PTH、CTX-1水平均降低,OC水平均升高,与干预前比较,均P <0.05,对照组25(OH) D水平低于干预前(P <0.05);且研究组Ca、P、PTH和CTX-1水平低于对照组,25(OH) D、OC水平和股骨骨密度T值高于对照组(P均<0.05)。结论右归丸可改善老年维持性血液透析脾肾两虚型CKD患者骨代谢和骨转化指标,提高患者骨密度,并可为改善患者矿物质异常增益。Objective To observe the effect of Yougui pill on mineral and bone metabolism in patients with chronic kidney disease( CKD) with syndrome of yang deficiency of spleen and kidney. Methods 78 elderly patients with CKD who were admitted to the department of nephrology in Shangqiu Municipal Hospital from June 2019 to July 2020 and undergo regular hemodialysis were randomly divided into control group and study group,with 39 cases in each group. Patients in both groups received hemodialysis 3 times a week combined with hemoperfusion once every 2 weeks,and were treated with conventional western medicine. On this basis,the study group was given oral Yougui pills. Both groups were treated for 3 months. The changes in mineral and bone metabolism indexes of the two groups were compared before and after the intervention. Results After 3 months of intervention,the levels of Ca,P,PTH,and CTX-1 in the two groups decreased,while the levels of OC increased. Compared with the pre-intervention( P < 0. 05),25( OH) D level of the control group was lower than before the intervention( P <0. 05). The levels of Ca,P,PTH and CTX-1 in the study group were lower than those in the control group,and the levels of 25( OH) D,OC and the femoral bone mineral density T value were higher than those in the control group( all P < 0. 05). Conclusion Yougui pill can improve the bone metabolism and bone transformation indexes in elderly maintenance hemodialysis patients with syndrome of yang deficiency of spleen and kidney CKD,increase the bone density of the patients,and improve the abnormal gain of minerals in patients.
