摘　　要：[目的]介绍角度定位器辅助侧位髓内钉固定股骨转子间骨折的手术技术和初步临床效果.[方法]2019年1月-2020年9月应用角度定位器辅助侧位股骨近端防旋髓内钉(proximal femoral nail anti-rotation,PFNA)固定治疗股骨转子间骨折40例,术前应用Mimics软件测量健侧股骨颈前倾角,患者术中侧卧位,连接牵引床,在角度定位器辅助下置入PFNA内固定.[结果]所有患者均顺利完成手术,无神经、血管损伤等严重并发症.置钉准确率100.00%,手术时间(57.53±16.25)min,术中透视次数(4.45±0.51)次,术中失血量(123.18±22.63)ml.所有患者均获随访,时间5~14个月,平均(8.32±1.17)个月,末次随访时髋关节Harris评分(81.23±6.17)分.[结论]应用股骨颈前倾角角度定位器辅助侧位髓内钉固定,在缩短手术时间及X线曝光次数方面有明显的优势.[Objective]To introduce the surgical technique and preliminary clinical outcomes of an angle locator assisted proximal femoral nail anti-rotation(PFNA)for internal fixation of femoral intertrochanteric fractures.[Methods]From January 2019 to September 2020,a total of 40 patients were treated with PFNA assisted with a self-developed angle locator.Preoperatively,Mimics software was used to measure the femoral anteversion on the healthy femoral neck.During the operation,the patient was placed in lateral decubitus position.The angle locator was connected to the traction bed,and guided the placement of PFNA in proper femoral anteversion.[Results]All the patients were successfully operated on without serious complications,such as nerve and vascular injury,whereas with accuracy of screw placement of 100%,operative time of(57.53±16.25)min,intraoperative fluoroscopy of(4.45±0.51)times,intraoperative blood loss of(123.18±22.63)ml.All patients were followed up for 5〜14 months,with a mean of(8.32±1.17)months,and the Harris hip function score at the latest follow-up was(81.23±6.17).[Conclusion]This angle locator assisted PFNA has the advantages of shortening the operation time and decreasing X-ray exposure times for femoral intertrochanteric fractures.
