梅毒逆向筛查策略在出入境人员梅毒监测中的应用分析

摘　　要：本文对宁波口岸2006—2020年出入境人员梅毒监测情况进行了分析,探讨梅毒逆向筛查策略在出入境人员梅毒监测中的应用价值。采用梅毒螺旋体明胶颗粒凝集试验(TPPA)定性检测,对申请出入境健康体检人员进行梅毒抗体初筛,阳性人员以甲苯胺红不加热血清学试验(TRUST)复测,双阳人员加做TPPA及TRUST定量滴度检测。分析结果显示,101198例出入境人员TPPA初筛阳性率1.19%,出境人员阳性检出率高于入境人员,男女性别比为7.1:1,40~49岁年龄组检出率最高;TRUST阳性检出率0.36%,与TPPA阳性的符合率30.27%;TRUST阳性检出人数随滴度升高而减少,TPPA阳性检出呈现滴度越高检出人数越多的趋势。结果表明,以TPPA为初筛的梅毒逆向筛查策略可以在较短时间内从出入境人员中检出更多梅毒疑似感染者。The paper analyzes the syphilis surveillance in entry-exit personnel at Ningbo port from 2006 to 2020,and discusses the application of reverse syphilis screening strategy.Treponema pallidum particle agglutination test(TPPA)was used to conduct preliminary screening of syphilis antibody in entry-exit health examination applicants.The positive applicants were tested again with toluidine red unheated serum test(TRUST).Applicants with TPPA and TRUST testing both positive were tested quantitative titers for TPPA and TRUST.The results showed that TPPA positive rate in 101198 entry-exit people was 1.19%,and TRUST positive rate was 0.36%,coincidence rate with TPPA was 30.27%.TPPA positive rate in exit people was higher than that of entry people,and the male to female ratio was 7.1:1,and 40-49 age group was the highest in whole age groups.The number of TRUST positive tests decreased with the increase of titer,while TPPA positive tests showed a trend of higher titer accompanying more detected people.In conclusion,the reverse syphilis screening strategy based on TPPA can detect more suspicious persons within a short period of time.
