复方真武冲剂治疗老年慢性心衰患者的临床疗效观察

摘　　要：目的:探讨复方真武冲剂联合沙库巴曲缬沙坦治疗老年心肾阳虚型慢性心力衰竭的临床疗效;方法:从2019年1月—2021年1月安徽省中医药大学第一附属医院老年病中心心血管内科患者中选取符合纳入标准的老年心肾阳虚型慢性心衰患者72例,按照随机分组原则分为对照组和治疗组各36例。对照组予新型药物沙库巴曲缬沙坦,治疗组加用复方真武冲剂,疗程为3个月。评估2组患者干预前后中医症状积分、明尼苏达心衰量表积分(Minnesota heart failure quality of life,MLHFQ)、6min步行试验距离(6-minute walk test distance,6MWD),比较2组患者干预前、后心力衰竭指标氨基酸末端脑钠素前体(N-terminal pro-B-type natriuretic peptide,NT-proBNP)含量、心脏超声指标左室射血分数(the ejection fraction of left ventricular,LVEF)以及全血高切黏度、全血低切黏度、血浆比黏度及红细胞聚集指数(EAI)等血液流变学指标。结果:与治疗前比较,2组均能降低中医症状、血流变指标、MLHFQ 、NT-proBNP含量及提高6MWD、LVEF;与对照组相比,治疗组在降低中医症状、血流变指标、MLHFQ、NT-proBNP含量及提高6MWD、LVEF更明显,差异有统计学意义。结论:在治疗老年心肾阳虚型慢性心衰疗效方面,复方真武冲剂联合沙库巴曲缬沙坦较仅用后者更为显著,联合治疗则可通过有效改善血流变指标从而达到改善临床症状、提高患者运动耐力、改善心功能以提高患者生活质量水平,值得进一步研究及推广。Objective:To explore the clinical efficacy of compound Zhenwu granules combined with sacubitril and valsartan in the treatment of chronic heart failure with heart and kidney yang deficiency in the elderly;method:from January 2019 to January 2021 The First Affiliated Hospital of Anhui University of Traditional Chinese Medicine In the Department of Cardiology of the Geriatrics Center,72 elderly patients with chronic heart failure of heart and kidney yang deficiency type who met the inclusion criteria were selected,and were divided into a control group and a treatment group with 36 cases each according to the principle of randomization.The control group was given the new drug sacubatril and valsartan,and the treatment group was given compound Zhenwu granules for 3 months.To evaluate the scores of TCM symptoms,Minnesota heart failure quality of life (MLHFQ),6-minute walk test distance (6MWD) before and after intervention in the two groups of patients,and compare the two groups before and after intervention Heart failure index amino acid terminal pro-B-type natriuretic peptide (NT-proBNP) content,cardiac ultrasound index left ventricular ejection fraction (the ejection fraction of left ventricular,LVEF) and whole blood hyperemia Shear viscosity,low shear viscosity of whole blood,plasma specific viscosity and erythrocyte aggregation index (EAI) and other hemorheological indicators.Results:Compared with before treatment,both groups can reduce TCM symptoms,blood rheology indexes,MLHFQ,NT proBNP content,and increase 6MWD and LVEF.Compared with the control group,the treatment group is reducing TCM symptoms,blood rheology indexes,The content of MLHFQ,NT-proBNP and the increase of 6MWD and LVEF were more obvious,and the difference was statistically significant.Conclusion:In the treatment of chronic heart failure with heart and kidney yang deficiency in the elderly,the compound Zhenwu granules combined with sacubitril and valsartan are more significant than the latter alone,and the combined treatment can effectively improve t
