摘 要:目的构建一个经济、有效、系统与规范的脑卒中鼻饲患者围拔胃管期营养护理方案,为脑卒中鼻饲患者早期拔除胃管,实现持续营养管理提供依据。方法基于文献复习和指南回顾,经专家小组讨论,初步形成“脑卒中鼻饲患者围拔胃管期营养护理方案”,运用德尔菲(Delphi)法进行专家咨询,对函询结果进行统计分析,最终完成方案构建。结果本研究邀请浙江省内13位专家,共开展2轮专家函询,每轮专家咨询问卷有效回收率100%,方案整体问卷权威系数分别为0.83和0.89,2轮专家的肯德尔系数分别为0.231和0.118(P<0.05);最终方案共有3个一级指标、22个二级指标和36个三级指标,主要包括“吞咽功能锻炼、过渡期饮食训练及心理支持”3方面内容。结论“脑卒中鼻饲患者围拔胃管期营养护理方案”具有权威性、科学性和实用性,本研究结果可以为“脑卒中鼻饲患者围拔胃管期营养护理”提供干预措施依据。Objective To develop an economical,effective,systematic and standardized nutritional care scheme for stroke nasal feeding patients during removing gastric tube,so as to provide a basis for the early removal of the gastric tube and continuous nutritional management of stroke nasal feeding patients.Methods Based on the literature review and guideline review,the"nutritional care scheme for patients with stroke and nasal feeding during removing gastric tube"was initially developed by expert group discussion.Results In this study,13 experts from Zhejiang province were invited to conduct 2 rounds of expert consultation,and the effective return rate of the questionnaire in each round was 100%.The final protocol consisted of 3 level indicators,22 level 2 indicators and 36 level 3 indicators,including"swallowing function exercise,transitional diet training and psychological support".Conclusion The"nutritional care scheme for patients with stroke and nasal feeding during removing gastric tube"is authoritative,scientific and practical,and the results of this study can provide a basis for interventions in the"nutritional care scheme for patients with stroke and nasal feeding during removing gastric tube".
