摘　　要：目的了解北京地区医院对腹膜透析患者的营养管理现状,为做好腹膜透析患者规范的营养管理提供依据。方法自行设计调查问卷,采用便利抽样法对北京29家医院腹膜透析中心的管理人员进行调查。结果29家医院中有10家医院(34.5%)腹膜透析中心护士配置未达到《腹膜透析标准操作流程》要求;有18家医院(62.1%)开展腹膜透析患者的营养管理。18家开展营养管理的医院中有16家医院对患者进行营养状态评估,其中9家医院使用SGA评估工具;有12家医院进行饮食摄入评估,其中8家医院使用24小时回顾法,7家医院使用3天饮食日记。18家医院均进行蛋白摄入推荐和限盐限水指导,其中17家医院进行饮食能量摄入推荐指导,12家医院指导饮食日记的记录方法;各家医院指导的饮食蛋白摄入量不一,与相关指南推荐相比存在差距。结论目前北京地区各医院腹膜透析中心的人力配置总体不足、营养管理工作开展不尽人意,尚需进一步加强和规范。Objective To learn about the status quo of nutritional management of peritoneal dialysis patients in hospitals in Beijing,and to provide a basis for the standardized nutritional management of peritoneal dialysis patients.Methods Self-designed questionnaires,and convenience sampling method were used to survey the managers of 29 peritoneal dialysis centers in Beijing.Results Of the 29 hospitals,10 hospitals(34.5%)did not meet the requirements of the"Standard operating procedures for peritoneal dialysis"in the peritoneal dialysis centers.18 hospitals(62.1%)carried out nutritional management of peritoneal dialysis patients.Of the 18 hospitals that carried out nutrition management,16 hospitals assessed the nutritional status of patients,9 of which used SGA assessment tools.12 hospitals performed dietary intake assessments,8 of which used 24-hour retrospective methods,and 7 of them used 3-day diet diaries.18 hospitals recommended protein intake recommendations and limited salt and water restrictions,of which 17 hospitals recommended dietary energy intake recommendations,and 12 hospitals guided the recording methods of diet diaries.The dietary protein intakes guided by each hospital were different,and there was a gap compared with the recommendations of the relevant guidelines.Conclusion At present,the overall manpower allocation of peritoneal dialysis centers in hospitals in Beijing area is inadequate,and nutritional management work is unsatisfactory.It needs to be further strengthened and standardized.
