基于科室与患者特点的平均住院日管控分析

摘　　要：目的使用床位利用指数及DRG相关指标分析医院各科室住院患者特点,依据各科室实际情况,对平均住院日进行管控。方法选取2017年至2020年10月医院的统计年报数据以及经广东省CN-DRG分组得到的数据对不同科室床位使用效率、DRG入组情况、收治病种等进行统计分析。结果医院平均住院日从2017年5.67天降至2020年5.16天。妇科平均住院日4年间下降明显,CMI在0.9附近波动;产科平均住院日虽逐步下降,但高于广东省的平均水平;儿内科平均住院日维持在4.2天左右,且明显低于广东省的平均水平,但CMI一直在0.5附近波动,需提高诊疗水平,增加复杂病种的收治。结论管理部门应根据不同科室收治患者特点,针对性地制定改进措施,合理控制科室平均住院日。Objective The bed utilization index and DRG related indicators were used to analyze the characteristics of inpatients in various departments of the hospital.The average length of stay in accordance with the actual conditions of each department was managed and controlled.Methods The statistical annual report data of hospitals from 2017 to October 2020 and the data obtained by the CN-DRG grouping of Guangdong Province were selected to perform statistical analysis on the utilization efficiency of beds in different departments,the status of DRG enrollment,and the types of diseases admitted.Results The average length of stay dropped from 5.67 days in 2017 to 5.16 days in 2020.The average length of stay in gynecology has dropped significantly since the last 4 years,and the CMI fluctuated around 0.9.Although the average length of stay in obstetrics has gradually decreased,it was higher than the average in Guangdong Province.The average length of stay in pediatrics has remained at about 4.2 days,and was significantly lower than the average level in Guangdong Province,but the CMI has been fluctuating around 0.5.It is necessary to improve the level of diagnosis and treatment and increase the admission of complex diseases.Conclusion The management department should formulate targeted improvement measures according to the characteristics of patients admitted to different departments,and reasonably control the average length of stay of the departments.
