不同入路人工髋关节置换术治疗老年股骨颈骨折效果观察及其对血清骨特异性碱性磷酸酶抗酒石酸酸性磷酸酶5b水平的影响

摘　　要：目的探究不同入路人工髋关节置换术应用于老年股骨颈骨折患者的效果。方法选取我科123例行人工髋关节置换治疗老年股骨颈骨折患者,根据随机数字表法分组。对照组61例给予传统后外侧入路人工髋关节置换术,观察组62例给予前外侧入路人工髋关节置换术,对比2组患者手术指标、髋关节功能、血液学指标及术后并发症。结果观察组切口长度、手术时间、术中出血量、术后引流量、抗酒石酸酸性磷酸酶5b(TRACP5b)、脱氧吡啶酚(DPD)水平、并发症发生率均低于对照组,髋关节功能(Harris)各项评分、骨特异性碱性磷酸酶(BALP)水平高于对照组(P<0.05)。结论前外侧入路人工髋关节置换术对老年股骨颈骨折患者治疗,可有效改善骨吸收状态、髋关节功能,降低术中出血量、手术时间、术后引流量及术后并发症,安全性高。Objective To explore the effect of different approaches of hip arthroplasty in elderly patients with femoral neck fracture.Methods One hundred and twenty-three elderly patients with hip arthroplasty for femoral neck fracture were selected and divided into control group and observation group according to random number table.Sixty-one patients in the control group received traditional posterolateral hip arthroplasty,and 62 patients in the observation group received anterolateral hip arthroplasty.The surgical indicators,hip function,hematological indicators and postoperative complications of the two groups were compared.Results The incision length,operation time,intraoperative blood loss,postoperative drainage,the level of tartrate-resistant acid phosphatase isoform 5b(TRACP5b)and deoxypyridinoline(DPD)level and the incidence of complications in the observation group were lower than those in the control group,and Harris score and bone alkaline phosphatase(BALP)level were higher than those in the control group(P<0.05).Conclusion Anterolateral approach of artificial hip replacement for the treatment of elderly patients with femoral neck fracture can effectively improve the bone absorption state,hip function,and reduce the intraoperative blood loss,operation time,postoperative drainage and postoperative complications,which is with high safety.
