摘　　要：目的:研究不同频率推拿手法对中风后上肢痉挛性瘫痪患者运动功能的影响,从而为临床推拿治疗该病提供方法借鉴,推动规范化推拿治疗方案形成。方法:该研究随机将该院收住的符合纳入标准的80例患者分入治疗组(慢频率推拿组)与对照组(常规频率推拿组),每组40例,2组治疗方案相同,但推拿频率不同,2组均每周治疗6d,1次/d;2周1疗程,治疗2疗程后采用上肢简化Fugl-Meyer运动功能评分量表(FMA)、改良Ashworth分级法(其中0、1、1+、2、3、4分别对应0-5分,方便计算)评定疗效。结果:治疗2个疗程后,2组在上肢FMA评分、改良Ashworth分级评分较治疗前明显改善(P<0.01);治疗后慢频率推拿组(治疗组)疗效评价优于对照组(P<0.01)。结论:用力轻、时间长、慢频率、顺时针方向的推拿手法,可以有效抑制脑卒中后大脑神经过度兴奋,降低痉挛程度,改善运动功能,疗效显著,有利于推动推拿手法规范化。Objective:To study the influence of different frequency massage techniques on the motor function of patients with upper limb spastic paralysis after stroke,so as to provide reference for clinical massage treatment of the disease,and to promote the formation of standardized massage treatment programs.Method:The study randomly divided 80 patients admitted to the hospital into the treatment group(slow frequency massage group)and control group(routine frequency massage group),40 cases in each group,and the treatment plan of the two groups was the same,but Tuina frequency was different.Both groups were treated for 6 days per week,1 time/d;1 course of treatment for 2 weeks,after 2 courses of treatment,the simplified Fugl-Meyer motor function rating scale(FMA)and the modified Ashworth grading method were used(0,1,1+,2,3,4 correspond to 0-5 points respectively,which is convenient for calculation)to evaluate the curative effect.Results:After 2 courses of treatment,the upper limb FMA score and modified Ashworth grading score of the 2 groups were significantly improved compared to before treatment(P<0.01);after treatment,the slow frequency massage group(treatment group)had better efficacy evaluation than the control group(P<0.01).Conclusion:The massage technique with light force,long time,slow frequency and clockwise direction can effectively inhibit the excessive excitement of the cerebral nerves after stroke,reduce the degree of spasticity,improve motor function,and have a significant effect,which is conducive to promoting the standardization of massage techniques.
