提壶揭盖法联合针灸治疗卒中后神经源性膀胱临床疗效观察

摘　　要：目的:探究提壶揭盖法联合针灸治疗卒中后神经源性膀胱(PSNB)的临床疗效。方法:60例卒中后神经源性膀胱患者(符合纳入标准)随机分为治疗组和对照组,每组各30例。治疗组采用提壶揭盖法联合针灸+间歇导尿治疗,对照组采用核心肌群锻炼(盆底肌为主)+间歇导尿治疗。观察两组病人治疗前后的残余尿量、膀胱最大容量、NIHSS评分、每天排尿的次数、单次的排尿量、每天尿失禁的次数及临床疗效。结果:治疗组病人总有效率(93.33%,28/30)>对照组(70.00%,21/30)(P=0.045<0.05)。与治疗前比较,两组治疗后残余尿量、每天排尿的次数、每天尿失禁的次数、NIHSS评分均减少,膀胱的最大容量、单次的排尿量均增加,且治疗组的改善较对照组明显(P<0.05)。结论:提壶揭盖法联合针灸治疗PSNB临床疗效较好,且安全性高,无不良反应,可操作性强,可以进一步的推广应用。Objective:To explore the clinical efficacy of the method of lifting the pot and uncovering the lid combined with acupuncture in the treatment of post-stroke neurogenic bladder (PSNB).Methods:Sixty patients with neurogenic bladder after stroke (in line with the inclusion criteria) were randomly divided into treatment group and control group,with 30 cases in each group.The treatment group was treated with pot lifting and lid removal combined with acupuncture + intermittent catheterization,and the control group was treated with core muscle exercises (mainly pelvic floor muscles) + intermittent catheterization.Observe the residual urine volume,maximum bladder capacity,NIHSS score,number of urination per day,single urination volume,number of urinary incontinence per day,and clinical efficacy of the two groups of patients before and after treatment.Results:The total effective rate of patients in the treatment group (93.33%,28/30)>control group (70.00%,21/30) (P=0.045<0.05).Compared with before treatment,the residual urine volume,the number of urination per day,the number of urinary incontinence per day,and the NIHSS score were reduced after treatment in the two groups,and the maximum bladder capacity and single urination volume increased,and the treatment group improved compared with the control group Obviously (P<0.05).Conclusion:The method of lifting the pot and removing the lid combined with acupuncture and moxibustion has a good clinical effect,high safety,no adverse reactions,and strong maneuverability.It can be further promoted and applied.
