胸腺五肽辅助比阿培南对老年重症肺炎患者免疫功能的影响

摘　　要：目的:探讨胸腺五肽辅助比阿培南对老年重症肺炎患者免疫功能的影响。方法:选取2019年8月-2021年6月惠州市第三人民医院重症医学科收治的122例老年重症肺炎患者作为研究对象,并根据临床治疗方式不同将其分为对照组与观察组,每组61例。对照组采用比阿培南治疗,观察组采用胸腺五肽和比阿培南治疗。比较两组患者临床疗效、症状与体征改善时间、免疫功能以及不良反应发生情况。结果:治疗后,观察组总有效率(93.44%)高于对照组(80.33%)(P<0.05)。治疗后,观察组咳嗽咳痰缓解时间、肺啰音消失时间、肺部阴影消失时间、退热时间均短于对照组(P<0.05)。治疗后,观察组成熟T淋巴细胞(CD3+)、辅助/诱导T淋巴细胞(CD4+)、CD4+/CD8+均高于对照组,抑制/细胞毒T淋巴细胞(CD8+)低于对照组(P<0.05)。观察组不良反应发生率(4.92%)与对照组(8.20%)比较,差异无统计学意义(P>0.05)。结论:胸腺五肽辅助比阿培南可改善老年重症肺炎患者免疫功能,快速缓解症状、体征,进而提高临床疗效,且不增加不良反应发生率。Objective:To explore the effect of Thymopentin assisted Biapenem on the immune function of elderly patients with severe pneumonia.Method:A total of 122 elderly patients with severe pneumonia admitted to the Intensive Medicine Department of Huizhou Third People’s Hospital from August 2019 to June 2021 were selected as the research objects,and they were divided into the control group and the observation according to their clinical treatment methods,with 61 cases in each group.The control group was treated with Biapenem,and the observation group was treated with Thymopentin and Biapenem.The clinical efficacy,the time to improve symptoms and signs,the immune function and the occurrence of adverse reactions were compared between the two groups.Result:After treatment,the total effective rate(93.44%)of the observation group was higher than 80.33%of the control group(P<0.05).After treatment,the observation group’s cough and sputum relief time,pulmonary rales disappearance time,lung shadow disappearance time,and antipyretic time were shorter than those in the control group(P<0.05).After treatment,the mature T lymphocytes(CD3+),auxiliary/induced T lymphocytes(CD4+)and CD4+/CD8+in the observation group were higher than those in the control group,and the inhibitory/cytotoxic T lymphocytes(CD8+)was lower than that in the control group(P<0.05).The incidence of adverse reactions in the observation group(4.92%)and the control group(8.20%)were not significantly different(P>0.05).Conclusion:Thymopentin assisted Biapenem can improve the immune function of elderly patients with severe pneumonia,quickly alleviate symptoms and signs,then improve clinical efficacy without increasing the incidence of adverse reactions.
