糖尿病肾病患者血清纤维化指标与肾血流的关系探究

摘　　要：目的:探究与分析糖尿病肾病患者血清纤维化指标与肾血流的关系。方法:选取2018年6月-2020年12月沈阳医学院附属第二医院的100例糖尿病肾病患者为观察组,同时期的100名同龄健康体检者为对照组。比较两组的血清纤维化指标[透明质酸(HA)、层粘连蛋白(LN)及转化生长因子-β1(TGF-β1)]、肾段动脉及肾叶间动脉血流指标[最大血流速(Vmax)、最小血流速(Vmin)及阻力指数(RI)],同时比较不同糖尿病肾病分期患者的上述指标检测结果,采用Pearson相关性分析上述血清纤维化指标与肾血流指标的关系。结果:观察组的血清纤维化指标与肾段动脉及肾叶间动脉RI均显著高于对照组,肾段动脉及肾叶间动脉Vmax、Vmin均显著低于对照组,不同糖尿病肾病分期患者的上述指标检测结果比较,差异均有统计学意义(P<0.05);Pearson相关性分析显示,血清纤维化指标与肾段动脉及肾叶间动脉RI均呈正相关,与肾段动脉及肾叶间动脉Vmax、Vmin均呈负相关(P<0.05)。结论:糖尿病肾病患者血清纤维化指标与肾血流均显著异常,且其之间有密切的关系,因此在糖尿病肾病患者中的检测价值较高。Objective:To investigate and analyze the relationship between serum fibrosis indexes and renal blood flow of patients with diabetic nephropathy.Method:A total of 100 patients with diabetic nephropathy in the Second Affiliated Hospital of Shenyang Medical College from June 2018 to December 2020 were chosen as the observation group,100 healthy people of the same ages with physical examination at the same time were chosen as the control group.Then the serum fibrosis indexes[hyaluronic acid(HA),laminin(LN)and transforming growth factor-β1(TGF-β1)],renal segmental artery and interlobar artery blood flow indexes[maximum flow velocity(Vmax),minimum flow velocity(Vmin)and resistance index(RI)]of two groups were compared,the results of the above indicators were compared among different stages of diabetic nephropathy,then the correlation between those serum fibrosis indexes and renal blood flow indexes were analyzed with Pearson correlation analysis.Result:The serum fibrosis indexes and renal segmental artery and interlobar artery RI of observation group were significantly higher than those of control group,the renal segmental artery and interlobar artery Vmax and Vmin were all significantly lower than those of control group,and the detection results of observation group with different stages of diabetic nephropathy were compared,there were statistically significant differences(P<0.05);the Pearson correlation analysis showed the serum fibrosis indexes were positively correlated with renal segmental artery and interlobar artery RI,and they were negatively correlated with renal segmental artery and interlobar artery Vmax and Vmin(P<0.05).Conclusion:The serum fibrosis indexes and renal blood flow of patients with diabetic nephropathy are significantly abnormal,and they have close relationship,so the detection value in the patients with diabetic nephropathy is higher.
