甲状腺相关眼病的眼压特征及不同疗法对眼压的影响

摘　　要：目的研究甲状腺相关眼病(TAO)患者的眼压特征及不同治疗方法对眼压的影响。方法回顾性病例系列研究。回顾39例(55眼)TAO住院患者的临床资料,分析影响TAO患者眼压的因素及不同治疗方式对患者眼压的影响。结果 39例TAO患者中,20例(34眼51.28%)出现高眼压,其中眼压与患者的年龄、性别、既往是否表现甲状腺功能异常无关,与眼球突出度、CAS评分、眼外肌受累条数相关;激素治疗、斜视手术、眶减压手术均可改善高眼压组TAO患者的眼压,各组间治疗前后的眼压变化差异无明显统计学意义(P>0.05)。结论TAO患者眼压升高与眼球突出度、CAS评分、眼外肌受累条数相关,激素治疗、斜视手术、眶减压手术三种治疗方式均可改善TAO患者的眼压情况。Objective To study the characteristics of intraocular pressure in patients with thyroid-associated ophthalmopathy(TAO) and the effects of different treatments on intraocular pressure.Methods Retrospective case series study.39 hospitalized patients(55 eyes) with TAO were retrospectively analyzed.The general situation and factors affecting IOP and the effects of different treatments on IOP were analyzed.Results Among the 39 patients,20(51.28%)patients(34 eyes) had high intraocular pressure.Intraocular pressure was not related to age,gender and previous thyroid dysfunction,but related to exophthalmos,CAS score and the number of extraocular muscle involvement.Methylprednisolone therapy,strabismus surgery and orbital decompression surgery can improve the IOP of TAO patients in the high intraocular pressure group.There was no significant difference in the changes of intraocular pressure between the groups before and after treatment(P>0.05).Conclusions Elevated intraocular pressure is related to exophthalmos,CAS score and the number of extraocular muscle involvemen.Methylprednisolone therapy,orbital decompression and strabismus correction could improve the intraocular pressure of patients.
