基于家庭医生签约服务的社区老年2型糖尿病患者居家护理方案构建研究

摘　　要：目的在家庭医生签约服务的基础上,构建一套较为科学、实用的社区老年2型糖尿病患者居家护理方案。方法通过文献回顾、问卷调查、半结构式访谈及专家会议,确定基于家庭医生签约服务的社区老年2型糖尿病患者居家护理方案的框架及具体内容。结果参会专家判断依据系数为0.910,熟悉程度系数为0.820,权威系数为0.865,构建基于家庭医生签约服务的社区老年2型糖尿病患者居家护理方案内容包括居家护理方案的目标和宗旨、居家护理服务团队、居家护理服务流程及内容三部分。结论本研究构建的居家护理方案科学、合理、可靠,可为社区医疗机构开展老年2型糖尿病患者居家护理服务提供依据。Objective To construct a scientific and practical home nursing model based on Family Doctor service for Elderly Type 2 diabetes patients in Community.Methods Based on literature review,investigation,semi-structured interview and expert meeting,the framework and specific content of home nursing model of elderly type 2 diabetic patients based on family doctor service were constructed.Results The expert judgment coefficient was 0.910,the degree of expert familiarity was 0.820,and the degree of expert authority was 0.865.The home nursing model contained objective and purposes,service team,and service processes and content.Conclusion The home nursing model is logical,reliable and comprehensive,which can be used as reference for development of home care for elderly patients with type 2 diabetes in community.
