骨折创伤患者应用全自动凝血仪检测凝血功能的效果研究

摘　　要：目的探讨全自动凝血仪检测骨折创伤患者凝血功能的效果。方法选取2020年1月—2020年8月医院骨折创伤患者337例作为骨折创伤组,另回顾性选取同期本院健康体检人员50例作为健康对照组,采用迈瑞ExC810全自动凝血仪及其配套试剂对两组人员的血浆D-二聚体(D-Dimer)、纤维蛋白原降解产物(FDP)及凝血四项(纤维蛋白原-Fbg、凝血酶原时间-PT、凝血酶时间-TT、活化部分凝血活酶时间-APTT)进行测定。统计分析两组人员的血浆D-Dimer、FDP水平、凝血四项,并统计分析骨折创伤组患者治疗前后的凝血四项。结果骨折创伤组患者的血浆D-Dimer、FDP水平均高于健康对照组(t=25.793,34.744,P<0.05)。骨折创伤组患者的血浆Fbg水平高于健康对照组(t=24.316,P<0.05),但两组人员的PT、TT、APTT之间的差异均不显著(t=1.448,0.532,2.709,P>0.05)。骨折创伤组患者治疗后的血浆Fg水平低于治疗前(t=47.583,P<0.05),但两组人员的PT、TT、APTT之间的差异均不显著(t=0.494,0.851,0.339,P>0.05)。结论全自动凝血仪能有效检测骨折创伤患者凝血指标,中医治疗效果好。Objective To explore the effect of automatic coagulation instrument on detecting coagulation function in patients with fracture and trauma.Methods 337 patients with fracture and trauma in our hospital from January to August 2020 were retrospectively selected as the fracture and trauma group, and 50 healthy physical examination personnel in our hospital in the same period were retrospectively selected as the healthy control group. The plasma D-dimer, fibrinogen degradation product(FDP) and coagulation(fibrinogen Fbg Prothrombin time PT, thrombin time TT and activated partial thromboplastin time APTT) were measured. The levels of plasma D-dimer, FDP and coagulation in the two groups were statistically analyzed. The four items of coagulation before and after treatment in the fracture and trauma group were statistically analyzed.Results the levels of plasma D-dimer and FDP in fracture trauma group were higher than those in healthy control group(t=25.793, 34.744, P< 0.05). The level of plasma Fg in fracture trauma group was higher than that in healthy control group(t=24.316, P<0.05), but there was no significant difference in PT, TT and APTT between the two groups(t=1.448, 0.532, 2.709, P>0.05). The plasma FG level of fracture trauma group was lower than that before treatment(t=47.583,P<0.05), but there was no significant difference in PT, TT and APTT between the two groups(t=0.494, 0.851, 0.339, P>0.05).Conclusion automatic coagulation instrument is effective in detecting coagulation function in patients with fracture and trauma, Chinese medicine treatment effect is good.
