河北省石家庄地区肺炎住院儿童腺病毒流行病学及基因特征分析

摘　　要：目的了解石家庄地区住院肺炎儿童中腺病毒感染及遗传变异规律。方法收集2018—2020年河北省某儿童医院住院肺炎患儿的支气管肺泡灌洗液标本444例,采用多重PCR方法对常见呼吸道病原体进行筛查,腺病毒阳性的标本进行病毒分离及分型,分离的毒株扩增其Penton、Hexon和Fiber基因并进行分析。结果444例样本中腺病毒检出率为6.76%(30/444),感染主要集中在6岁以上学龄儿童,检出高峰为冬季。混合感染24例,与肺炎支原体混合感染最常见。腺病毒分型显示其型别主要为3型,共分离出8株腺病毒,基因进化分析没有发现相关变异。结论2018—2020年河北石家庄地区肺炎住院儿童腺病毒感染以3型为主,感染人群以学龄期儿童为主,检出高峰为冬季。Objective To understand the regularity of adenovirus(ADV)infection and genetic variation among hospitalized children with pneumonia in Shijiazhuang area.Methods The bronchoalveolar lavage fluid(BALF)specimens were collected from 444 cases who were hospitalized in a children's hospital of Hebei province from 2018 to 2020.Common respiratory pathogens were detected by multiplex RT-PCR.Virus isolation and genotyping were performed for ADV-positive specimens.The Penton,Hexon and Fiber genes were amplified and analyzed.Results Among 444 cases of pneumonia,the positive rate of ADV was 6.76%(30/444).ADV infection occurred mainly in school-age children,with the peaks of detection in winter.Twenty-four cases were mixed infection,mainly with Mycoplasma pneumoniae.Molecular typing of ADV showed that the dominant type was ADV type 3.Eight ADV strains were isolated,and no corresponding variants was found by phylogenetic analysis.Conclusions In Shijiazhuang area of Hebei province from 2018 to 2020,ADV in hospitalized children with pneumonia was mainly ADV type 3,and school-age children were mainly affected.The peak of detection was in winter.
