舟山市健康儿童手足口病病原体隐性感染率及其影响因素分析

摘　　要：目的分析舟山市健康儿童手足口病病原体隐性感染率及其影响因素。方法采用分层整群随机抽样的方法抽取近半年内无发热、疱疹、皮疹等病毒感染症状的健康儿童,采集研究对象肛拭子标本,并进行问卷调查,采用肠道病毒通用检测、荧光定量PCR法测定手足口病病原隐性感染情况。结果舟山市健康儿童手足口病病原体隐性感染率为19.32%,其中EV71隐性感染率为3.40%;CoxA16隐性感染率为4.56%;其他肠道病毒隐性感染率为11.36%;未检出和多种病毒混合感染。多因素Logistic回归分析显示,性别、年龄、采样时期、家庭有其他14岁以下儿童、所在班级近2周手足口病暴发疫情史是舟山市健康儿童手足口病肠道病毒隐性感染率的影响因素(P<0.05)。结论舟山市健康儿童手足口病病原体隐性感染率较高,应针对危险因素开展重点人群保护和健康教育等工作。Objective To analyze the recessive infection rate of hand,foot,and mouth disease(HFMD)pathogens and its influencing factors among healthy children in Zhoushan City.Methods Healthy children with no symptoms of viral infection such as fever,herpes and rash in recent six months were selected by stratified cluster random sampling method.Anal swab specimens of the subjects were collected and questionnaire survey was conducted.General detection of enterovirus and fluorescence quantitative PCR were used to determine the recess infection of HFMD pathogens.Results The recessive infection rate of HFMD pathogens in healthy children in Zhoushan City was 19.32%,among which the recessive infection rate of EV71 was3.40%.The recessive infection rate of CoxA16 was 4.56%.The recessive infection rate of other enterovirus was 11.36%.No mixed infection with multiple viruses was detected.Multivariate Logistic regression analysis showed that gender,age,sampling period,other children under 14 years old in the family,and the recent 2-week history of HFMD outbreak in the class were the influencing factors for the recessive infection rate of HFMD among healthy children in Zhoushan City(P<0.05).Conclusion The recessive infection rate of HFMD pathogens in healthy children in Zhoushan City is high.Therefore,protection of key population and health education should be carried out according to risk factors.
