摘　　要：目的探讨利多卡因联合肾上腺素在美容外科局部肿胀麻醉处理过程中的应用效果。方法选取2017年1月至2020年1月朝阳市第二医院接受局部肿胀麻醉处理的患者100例作为研究对象,按随机数字表法分为联合组与常规组,每组50例。常规组实施利多卡因干预,联合组则予以利多卡因联合肾上腺素干预。比较两组镇痛效果、血清疼痛指标、炎症介质以及应激反应指标水平。结果联合组疼痛分级为0级患者占比明显高于常规组,而疼痛分级为Ⅰ级、Ⅱ级患者占比明显低于常规组,差异有统计学意义(P<0.05)。术后24h时联合组血清P物质(SP)、神经肽Y(NPY)水平低于常规组(P<0.05)。术后24h时联合组血清白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)水平低于常规组(P<0.05)。术后24 h时联合组血清去甲肾上腺素(NE)、皮质醇(Cor)水平低于常规组(P<0.05)。结论利多卡因联合肾上腺素在美容外科局部肿胀麻醉处理过程中的应用效果显著,可发挥较为理想的镇痛作用,减轻炎症反应以及应激反应。Objective To study the application effect of lidocaine combined with epinephrine in local swelling anesthesia in cosmetic surgery.Methods From January 2017 to January 2020,a total of 100 patients who received local swelling anesthesia in the Cosmetic Surgery Department of Chaoyang Second Hospital of Liaoning Province were selected as the research objects and were divided into a combined group and a conventional group by the random number table method,with 50 cases in each group.The conventional group received lidocaine intervention and the combined group received lidocaine and epinephrine intervention.The differences of analgesia effect,serum pain index,inflammatory factor and stress response before and after operation were analyzed.Results The number class 0 joint pain was significantly higher than conventional group,and pain class forⅠ,Ⅱnumber was significantly lower than conventional group(all P<0.05).At 24 h after surgery,the serum SP and NPY levels of the combined group were lower than conventional group(all P<0.05).The serum levels of IL-6 and IL-1βin the combined group were lower than conventional group 24 h after the operation(all P<0.05).The serum NE and Cor levels of the combined group were lower than conventional group 24 h after surgery(all P<0.05).Conclusion Lidocaine combined with epinephrine is effective in local swelling anesthesia in cosmetic surgery.It can play an ideal analgesic role and reduce inflammatory and stress reactions in patients.
