摘 要:目的探讨地佐辛与舒芬太尼用于脊柱术后患者自控静脉镇痛的临床效果。方法选取2018年6月至2020年7月于苏州大学附属常熟医院行脊柱外科手术患者90例作为研究对象,按随机数字表法分为对照组(n=45)与观察组(n=45)。对照组予以舒芬太尼,观察组采用舒芬太尼加地佐辛,用于术后镇痛。比较术后12 h、24 h内两组舒芬太尼用量、镇痛泵有效按压次数、视觉模拟评分法(VAS)评分、舒适度评分(BCS)评分及不良反应发生率。结果术后12 h、24 h,观察组舒芬太尼用量少于对照组(P<0.05);术后12 h、24 h,观察组VAS评分低于对照组,BCS评分高于对照组高,差异有统计学意义(P<0.05);观察组不良反应发生率为6.67%,低于对照组的26.67%(P<0.05)。结论地佐辛联合舒芬太尼应用于骨科脊柱手术术后患者自控静脉镇痛中,可减少舒芬太尼用量,缓解疼痛,提高舒适度,降低不良反应发生率。Objective To investigate the clinical effect of desoxocin and sufentanil on postoperative patient controlled intravenous analgesia in patients undergoing orthopedic spinal surgery. Methods A total of 90 orthopedic patients undergoing spinal surgery in admitted to Changshu Hospital Affiliated to Suzhou University from June 2018 to July 2020 were selected as the research objects and were divided into a control group(n=45) and an observation group(n=45) according to random number table method. The control group was given sufentanil, and the observation group was given sufentanil plus dezocine for postoperative analgesia. The dosage of sufentanil, the number of effective analgesic pump compressions, the visual analogue scale(VAS) score,the comfort score(BCS) score, and the incidence of adverse reactions were compared between the 2 groups within 12 h and 24 h after surgery. Results 12 h and 24 h after surgery, the amount of sufentanil in the observation group was less than that in the control group(P<0.05);At 12 h and 24 h after surgery, the VAS score of the observation group was lower than that of the control group, and the BCS score was higher than that of the control group, the difference was statistically significant(P<0.05). The incidence of ADR in observation group was 6.67% lower than that in control group 26.67%(P<0.05). Conclusions Desocine combined with sufentanil can reduce the amount of sufentanil, relieve pain, improve comfort, and reduce the incidence of adverse reactions in postoperative patient controlled intravenous analgesia for patients undergoing orthopedic spinal surgery.
