中药黄芪治疗糖尿病肾病患者的有效性及对患者血肌酐、尿素氮、24 h尿蛋白、尿β;-微球蛋白的影响

摘　　要：目的探讨糖尿病肾病患者采用中药黄芪治疗的有效性及对患者血肌酐、尿素氮、24h尿蛋白、尿β2-微球蛋白(β;-MG)的影响。方法选取2018年2月至2020年3月盘锦市中心医院收治的84例糖尿病肾病患者作为研究对象,按随机数字表法分为常规组(n=42)与黄芪组(n=42)。常规组采用常规治疗,黄芪组在常规组基础上采用复方黄芪汤治疗。比较两组疗效和治疗前后血糖(餐后2 h血糖(2hPBG)、糖化血红蛋白(HbA1c))、肾功能(血肌酐(Scr)、尿素氮(BUN)、24 h尿蛋白、尿β;-MG,炎症介质白细胞介素-1β(IL-1β)、C反应蛋白(CRP)。结果黄芪组治疗有效率92.86%(39/42)较常规组73.81%(31/42)高(P<0.05);治疗后,与常规组比较,黄芪组2hPBG、HbA1c水平较低,(8.76±1.12)mmol/L比(7.65±1.03)mmol/L,(6.91±0.52)%比(6.08±0.45)%,差异有统计学意义(P<0.05);治疗后,与常规组比较,黄芪组Scr、BUN、24 h尿蛋白、尿β;-MG水平较低,(68.16±7.28)μmol/L比(54.28±6.49)μmol/L,(6.61±0.84)mmoL/L比(5.06±0.75)mmoL/L,差异有统计学意义(P<0.05);治疗后,与常规组比较,黄芪组血清IL-1β、CRP水平较低,(1.82±0.29)μg/L比(1.15±0.21)μg/L,(7.26±1.19)mg/L比(5.98±1.03)mg/L,差异有统计学意义(P<0.05)。结论以中药黄芪为主的复方黄芪汤,应用于糖尿病肾病患者治疗中效果显著,可减轻炎症反应,降低血糖水平,改善肾功能。Objective To explore the clinical effect of Radix Astragali seu Hedysari on patients with diabetic nephropathy and its effects on patient’s serum creatinine, urea nitrogen, 24 h urine protein, urine β;-microglobulin(β;-MG). Methods A total of 84 patients with diabetic nephropathy from February 2018-March 2020 were selected as the research objects and were divided into a routine group(n=42) and a Radix Astragali seu Hedysari group(n=42). The conventional group was treated with conventional treatment, and the Radix Astragali seu Hedysari group was treated with compound Huangqin Decoction plus conventional treatment. The efficacy and blood glucose(2 h postprandial blood glucose(2 hPBG), glycosylated hemoglobin(HbA1 c), renal function(serum creatinine(Scr), urea nitrogen(BUN), 24 h urine protein, urine β;-MG), inflammatory mediators(interleukin-1β(IL-1β), C-reactive protein(CRP) were compared between the 2 groups before and after treatment. Results The total effective rate of Radix Astragali seu Hedysari group was 92.86%(39/42) higher than that of conventional group 73.81%(31/42)(P<0.05);After treatment, the level of 2 hPG、Hb A1 c in Radix Astragali seu Hedysari group was lower than that in conventional group(P<0.05);After treatment, The levels of Scr、BUN、24 h urine protein and urine β;-MG in Radix Astragali seu Hedysari group were lower than those in conventional group(P<0.05);After treatment,the serum IL-1β、CRP level of Radix Astragali seu Hedysari group was lower than that of conventional group(P<0.05). Conclusion The traditional Chinese medicine compound Huangqin Decoction, in the treatment of patients with diabetic nephropathy, can reduce inflammatory response, lower blood sugar levels, and improve renal function.
