摘　　要：目的探讨中药丹参川芎嗪注射液联合美托洛尔缓释片治疗冠状动脉粥样硬化性心脏病经皮冠状动脉介入术后慢性心力衰竭患者的疗效及不良反应。方法选取大连市中医医院2018年12月至2020年3月收治的冠状动脉粥样硬化性心脏病经皮冠状动脉介入术后慢性心力衰竭患者75例作为研究对象,按照不同的治疗方案分为对照组(34例)与观察组(41例)。对照组仅给予美托洛尔缓释片治疗,观察组联合丹参川芎嗪注射液治疗,比较两组临床疗效、左室收缩末期容量(LVESV)、左室舒张末期压力(LVEDP)、左室射血分数(LVEF)及血清脂联素(APN)、氧化型低密度脂蛋白(ox-LDL)水平和不良反应。结果观察组治疗有效率为95.12%,显著高于对照组76.74%,差异有统计学意义(P<0.05);治疗后,观察组LVESV、LVEDP分别为(91.01±31.93)ml/m^(2)、(17.23±2.04)mm Hg,显著低于对照组(111.21±28.20)ml/m^(2)、(21.75±2.01)mm Hg,观察组LVEF为(43.52±6.17)%,显著高于对照组(37.83±5.36)%,差异有统计学意义(P<0.05);治疗后,观察组血清APN及ox-LDL水平显著低于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论中药丹参川芎嗪注射液联合美托洛尔缓释片治疗冠状动脉粥样硬化性心脏病经皮冠状动脉介入术后慢性心力衰竭疗效显著,可有效改善患者心功能及血清APN、ox-LDL水平,且安全性较高。Objective To explore the efficacy and side effects of traditional Chinese medicine Danshen Ligustrazine Injection combined with Metoprolol Sustained Release Tablets in the treatment of patients with chronic heart failure after percutaneous coronary intervention for coronary atherosclerotic heart disease. Methods A total of 75 patients with chronic heart failure after percutaneous coronary intervention for coronary heart disease were selected as the research objects and were divided into a control group and an observation group according to different treatment plans. According to different treatment plan, the control group(n=34) was only treated with metoprolol sustained-release tablets, and the observation group(n=41) was treated with Danshen ligustrazine injection. The clinical efficacy, left ventricular end systolic volume(LVESV), left ventricular end diastolic pressure(LVEDP), left ventricular ejection fraction(LVEF), serum adiponectin(APN), and oxidized low-density lipoprotein(ox-LDL)levels and adverse reactions were compared between the 2 groups. Results The effective rate of the observation group was 95.12%, which was significantly higher than 76.74% of the control group, the difference was statistically significant(P<0.05);after treatment, the LVESV and LVEDP of the observation group were(91.01±31.93) ml/m;,(17.23±2.04) mm Hg, which is significantly lower than the control group(111.21±28.20) ml/m;,(21.75±2.01) mm Hg. LVEF of the observation group was(43.52±6.17)%,significantly higher than the control group(37.83±5.36)%, the differences were statistically significant(P<0.05);after treatment,the levels of APN and ox-LDL in the observation group were significantly lower than the control group, the difference is statistically significant(P<0.05). The incidence of side effects between the 2 groups was not statistically significant(P>0.05).
