摘  要:目的对比分析导流杂交基因芯片技术flow-through hybridization and gene chip, HybriMax)和多重PCR(multiplex PCR assays)两种方法对宫颈脱落细胞HPV DNA检测效果及一致率。方法通过HybriMax检测筛选2021-06—2021-09于解放军总医院第七医学中心妇科门诊就诊的100例阳性样本及32例阴性样本,并用多重HPV PCR检测试剂盒对这132例样本进行重复实验,并同时行液基薄层细胞学检测和DNA倍体检测,其中52例患者进行阴道镜下宫颈活检。结果对比HybriMax和多重PCR检测,HPV 59型、68型、81型一致性为较强,Kappa值分别为0.796、0.792、0.655,其他型别一致性均为强;多重PCR阳性型别检出数略高于HybriMa,相比于多重PCR方法,HybriMax敏感性部分型别较低,特异性均较高,其中HPV 51型、59型、68型和81型的敏感性分别为75%、66.7%、66.7%、60%;两种检测方法对比液基薄层细胞学、DNA倍体分析、宫颈活检结果一致性均为强和较强,Kappa值为0.621~0.975。结论 HybriMax检测和多重PCR两种方法均可用于宫颈癌筛查,其中,多重PCR阳性型别检出数略高于HybriMax,部分型别HybriMax敏感性较低。检测者可根据不同需要选择不同检测方法。Objective We evaluated the performance and consistency of two HPV DNA tests in cervical exfoliated cells, namely, flow-through hybridization and gene chip(HybriMax) and multiplex HPV PCR assays. Methods A total of 100 positive samples and 32 negative samples were selected in the Seventh Medical Center of PLA General Hospital from June 2021 to September 2021,which involves a two-test process of initial screening by HybriMax assays followed by multiplex HPV PCR assays. At the same time, thinprep cytologyic test(TCT)and DNA ploidy were performed. Among them, 52 patients underwent colposcopic cervical biopsy and pathology. Results Comparing of HybriMax and multiplex PCR assay, the consistency of HPV 59, 68 and 81 was strong, and the Kappa values were 0.796, 0.792 and 0.655, respectively, The other types was great strong. A slightly higher number of samples tested positive by the HPV multiplex PCR than by the HybriMax assay.HybriMax has lower sensitivity partly and both the two method have high specificity.The sensitivities of the HybriMax assay relative to those of the multiplex PCR assay for HPV types 51, 59, 68, and 81 were 75%、66.7%、66.7% and 60% respectively.The two method of HPV DNA assays compared to TCT, DNA ploidy and cervical biopsy result demonstrated a significant consistency with Kappa values ranging from 0.621 to 0.975. ConclusionBoth HybriMax and multiplex PCR can be effectively used for cervical cancer screening, Compared with multiple PCR method, HybriMax has lower sensitivity partly and numbers of samples tested positive. Different detection method can be chosen according to different needs.
