超低出生体质量早产儿晚发败血症短期预后的影响因素分析

摘　　要：目的探讨超低出生体质量(ELBW)早产儿晚发败血症(LOS)短期预后的影响因素。方法抽取2018年1月至2020年12月南阳医学高等专科学校第一附属医院收治的140例ELBW早产儿LOS患儿,全部患儿均接受相同治疗方案,依据预后分为预后不良组(病死,24例)、预后良好组(存活,116例),统计并比较两组基线资料和实验室指标,并分析短期预后不良的影响因素。结果预后不良组合并化脓性脑膜炎占比高于预后良好组(P<0.05),机械通气时间、抗生素使用时间长于预后良好组(P<0.05),降钙素原(PCT)水平高于预后良好组(P<0.05);组间其他基线资料、实验室指标比较,P>0.05;COX回归分析结果显示,合并化脓性脑膜炎、机械通气时间长、抗生素使用时间长及PCT水平高均是ELBW早产儿LOS患儿短期预后不良的影响因素(HR>1,P<0.05)。结论ELBW早产儿LOS患儿短期预后情况受合并化脓性脑膜炎、机械通气时间、抗生素使用时间及PCT水平等多种因素的影响。Objective To investigate the influencing factors of the short-term prognosis of late-onset sepsis(LOS)in extremely low birth weight(ELBW)premature infants.Methods A total of 140 cases of ELBW premature infants with LOS admitted to the First Affiliated Hospital of Nanyang Medical College from January 2018 to December 2020 were prospectively selected,all patients received the same treatment regimen,according to prognosis,they were divided into poor prognosis group(death,24 cases)and good prognosis group(survival,116cases),the baseline data and laboratory parameters were counted and compared between the two groups,and the factors affecting the short-term poor prognosis were analyzed.Results The proportion of combined purulent meningitis in poor prognosis group was higher than that in good prognosis group(P<0.05),the duration of mechanical ventilation and the use of antibiotics were longer than those in the good prognosis group(P<0.05),and the procalcitonin(PCT)level was higher than that in the good prognosis group(P<0.05);there was no statistical significant difference in other baseline data and laboratory indexes between the two groups(P>0.05);by Cox regression analysis,the results showed that combined purulent meningitis,long duration of mechanical ventilation,long duration of antibiotic use and high level of PCT were the influencing factors of poor short-term prognosis of ELBW premature infants with LOS(HR>1,P<0.05).Conclusions The short-term prognosis of ELBW premature infants with LOS is affected by multiple factors such as combined purulent meningitis,duration of mechanical ventilation,duration of antibiotic use and PCT level.
