摘  要:目的分析慢性阻塞性肺疾病急性加重期(AECOPD)患者血清转化生长因子-β1(TGF-β1)、白细胞介素-13(IL-13)水平对无创机械通气治疗预后的预测价值。方法抽取2019年1月至2020年12月商丘市第一人民医院收治的AECOPD患者110例,均接受无创机械通气治疗7 d,根据患者治疗7天内的预后情况分为预后良好组及预后不良组(转有创机械通气或病死),比较两组患者的基线资料及无创机械通气治疗前血清TGF-β1、IL-13水平,分析血清TGF-β1、IL-13水平对无创机械通气治疗预后的预测效能。结果110例中,预后良好组94例;预后不良组16例(14.55%),其中病死1例,转有创机械通气治疗15例。预后不良组血清TGF-β1、IL-13水平高于预后良好组(t=2.889、3.346,P<0.05)。两组间其他基线资料比较均无统计学差异(P均>0.05)。绘制受试者工作特征曲线,血清TGF-β1、IL-13水平预测预后不良风险的AUC>0.70。结论AECOPD患者血清TGF-β1、IL-13水平对无创机械通气治疗预后有一定的预测价值。Objective To analyze the value of serum transforming growth factor-β1(TGF-β1)-β1,interleukin-13(IL-13)levels in predicting prognosis of noninvasive mechanical ventilation on patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods 110 patients with AECOPD treated in Shangqiu First People’s Hospital from January 2020 to December 2020 were selected,all patients were treated with noninvasive mechanical ventilation,continuously for 7 days.According to the prognosis of patients within 7 days of treatment,they were divided intoa good prognosis group anda poor prognosis group(converted to invasive mechanical ventilation or death from illness).The baseline data questionnaire and the serum TGF-β1,IL-13 levels before noninvasive mechanical ventilationtreatment werecompared between the two groups.The correlation between the levels of serum TGF-β1,IL-13 and prognosis of noninvasive mechanical ventilation treatment was analyzed.Results Among 110patients,16 patientswere included into the poor prognosis group(14.55%),the other 94 cases were included into the good prognosis group.The levels of TGF-β1 and IL-13 in poor prognosis group were higher than those in good prognosis group(P<0.05);there was no statistical significant difference in other baseline data between the two groups(all P>0.05);The receiver operating curve was drawn,the AUC of serum TGF-β1 and IL-13 levels before noninvasive mechanical ventilation treatment in predicting the risk of poor prognosis in noninvasive mechanical ventilation in the treatment ofAECOPD patients was>0.70.Conclusions Serum TGF-β1 and IL-13 levels can predict the prognosis of noninvasive mechanical ventilation on patients with AECOPD.
