摘  要:目的探讨双氢睾酮(DHT)对老年前列腺增生(BPH)患者前列腺间质细胞炎症的影响。方法收集2016年1月至2017年5月就诊于内蒙古医科大学附属医院泌尿外科20例行经尿道前列腺电切术患者资料。采用组织块方法培养前列腺原代间质细胞。免疫组织细胞化学染色法鉴定所培养出的第3代基质细胞角蛋白、结蛋白、波形蛋白、α-平滑肌肌动蛋白(α-SMA)的表达。将第3代细胞传代于相同大小的培养皿,依次命名为空白组、对照组、实验组,加入相同浓度、剂量的培养液,空白组行细胞换液,对照组和实验组分别加入肿瘤坏死因子-α(TNF-α)10 ng/ml,5 h后在实验组加入DHT 30 nmol/L,再共同培养24 h,分别取三组上清液100μl,每组4个复孔。采用酶联免疫吸附试验法检测各组细胞因子:白细胞介素(IL)-6、IL-8、单核细胞趋化蛋白-1(MCP-1)和碱性成纤维细胞生长因子(bFGF)浓度水平变化。结果体外细胞实验培养出实验所需的原代前列腺间质细胞。免疫组织化学法鉴定第3代前列腺基间细胞:Cytokeratin、Desmin呈阴性表达;Vimentin、α-SMA呈阳性表达。对照组IL-6、IL-8、MCP-1、bFGF浓度高于空白组,差异均有统计学意义(P<0.05),实验组IL-6、IL-8、MCP-1、bFGF浓度低于对照组,差异均有统计学意义(P<0.05)。结论DHT对老年BPH患者的前列腺间质细胞具有直接抗炎作用。Objective To investigate the effect of Dihydrotestosterone(DHT)on prostatic stromal cell inflammation in aged benign prostatic hyperplasia(BPH)patients.Methods A total of 20 patients who underwent transurethral resection prostate in the Department of Urology,Affiliated Hospital of Inner Mongolia Medical University from January 2016 to May 2017 were collected.Primary prostatic stromal cells were cultured by tissue block method.The expression of Cytokeratin,Desmin,Vimentin andα-smooth muscle actin(α-SMA)in cultured stromal cells of the third generation were determined by immunohistochemical staining.The third generation of cells were cultured in the same size petri dishes and named as blank group,control group and experimental group.Culture medium with the same concentration and dose was added to the blank group.The control group and experimental group were added with tumor necrosis factor-α(TNF-α)10 ng/ml,30 nmol/L DHT was added in the experimental group 5 h later.Then co-culture for 24 h,take 100μl supernatant from three groups,with 4 rewells in each group.The concentrations of interleukin(IL)-6,IL-8,monocyte chemotactic protein-1(MCP-1)and basic fibroblast growth factor(bFGF)in each group were detected by enzyme-linked immunosorbent assay.Results Primary prostatic interstitial cells were cultured in vitro.Cytokeratin and Desmin were negatively expressed in the third generation of prostatic interbasal cells.Vimentin andα-SMA were positive.The concentrations of IL-6,IL-8,MCP-1,and bFGF in the control group were higher than those in the blank group,the differences were statistically significant(P<0.05),while the concentrations of IL-6,IL-8,MCP-1 and bFGF in the experimental group were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion DHT has a direct anti-inflammatory effect on prostatic interstitial cells in aged patients with prostatic hyperplasia.
