褥式缝合法与棒球式缝合法在子宫肌瘤腹腔镜手术患者中的应用效果

摘　　要：目的比较褥式缝合法与棒球式缝合法在子宫肌瘤腹腔镜手术患者中的应用效果。方法选取平顶山市妇幼保健院2017年10月至2019年10月接受腹腔镜子宫肌瘤剔除术的160例患者为研究对象,按照随机数表法分为两组,其中对照组80例患者术后采用传统连续褥式缝合法缝合,观察组80例患者术后采用棒球式缝合法缝合。对比两组患者围手术期指标、妊娠状况、性激素水平以及术后恢复情况。结果观察组缝合时间、手术时间短于对照组,术中失血量少于对照组,差异有统计学意义(P<0.05)。观察组不良妊娠率(11.25%)低于对照组(23.75%),差异有统计学意义(P<0.05)。术后1个月两组卵泡刺激素(FSH)、黄体生成素(LH)水平均较术前升高,观察组低于对照组,雌二醇(E_(2))均较术前降低,观察组高于对照组,差异有统计学意义(P<0.05)。观察组术后引流量小于对照组,排气时间、下床活动时间及住院时间均短于对照组,差异有统计学意义(P<0.05)。结论相较于传统连续褥式缝合法,棒球式缝合法可缩短缩短手术时间,减少术中和术后出血量,对患者术后妊娠情况以及性激素水平影响较小,有利于早期康复。Objective To compare the application effect of mattress suture and baseball suture in patients with laparoscopic surgery.Methods A total of 160 patients who underwent laparoscopic myomectomy in Maternal and Child Health Hospital of Pingdingshan from October 2017 to October 2019 were selected as the research objects and randomly divided into two groups,80 patients in the control group were sutured with traditional continuous mattress suture,and 80 patients in the observation group were sutured with baseball suture.The perioperative indicators,pregnancy status,sex hormone levels and postoperative recovery of the two groups were compared.Results The suture time,operation time and intraoperative blood loss of the observation group were less than those of the control group(P<0.05).The adverse pregnancy rate of the observation group(11.25%)was lower than the control group(23.75%)(P<0.05).The follicle stimulating hormone(FSH)and luteinizing hormone(LH)levels of both groups increased at one month after surgery compared with preoperative,the observation group was lower than the control group,estradiol(E_(2))was lower compared with preoperative,and the observation group higher than the control group(P<0.05).The postoperative drainage volume,exhaust time,ambulation time and hospitalization time of the observation group were less than those of the control group(P<0.05).Conclusion Compared with the traditional continuous mattress suture,baseball suture can shorten the operation time of laparoscopic myomectomy,reduce intraoperative and postoperative blood loss,and has less impact on postoperative pregnancy and sex hormone level,which is conducive to early rehabilitation.
