RPH-4三点套扎术联合聚桂醇硬化注射术与吻合器痔上黏膜环切术手术治疗出血性内痔的临床疗效比较

摘　　要：目的:探讨RPH-4三点套扎术联合聚桂醇硬化注射术对比吻合器痔上黏膜环切术(PPH)手术治疗出血性内痔的临床疗效及安全性。方法:48例出血性内痔患者随机分为治疗组与对照组,每组各24例。治疗组给予RPH-4三点套扎术联合聚桂醇硬化注射术,对照组给予PPH手术治疗,观察两组有效率及并发症的发生率。结果:治疗组有效率为100%,对照组有效率为95.83%,两组有效率比较,差异无统计学意义(P>0.05)。治疗组手术时间、术中出血量、平均住院日均小于对照组,差异有统计学意义(P<0.05),且术后疼痛、术后出血、术后尿潴留、术后肛门坠胀等并发症的发生率均明显低于对照组,差异有统计学意义(P<0.05)。结论:RPH-4三点套扎术联合硬化注射术治疗出血性内痔的有效率与PPH相当,但操作更简便,更微创,可缩短住院时间,有效减少术后各类并发症的发生。Objective To investigate the clinical efficacy and safety of RPH-4 three-point ligation combined with lauromacrogol sclerotherapy and PPH surgery for hemorrhagic internal hemorrhoids.Method Forty-eight patients with hemorrhagic internal hemorrhoids were randomly divided into two groups:treatment group and control group,with 24 cases in each group.The treatment group was given RPH-4 three-point ligation combined with lauromacrogoll sclerotherapy,and the control group was given PPH surgery,then the effectiveness and the incidence of complications of the two groups were observed.Results The effective rate was 100%in the treatment group and 95.83%in the control group.There was no significant difference in the effective rate between the two groups(P>0.05).The operation time,intraoperative blood loss,and average hospitalization days were lower in the treatment group than in the control group(P<0.05),and the incidence of complications such as postoperative pain,postoperative bleeding,postoperative urinary retention,and postoperative anal bulge were significantly lower than the control group(P<0.05).Conclusion The efficacy of RPH-4 three-point ligation combined with lauromacrogol for hemorrhagic internal hemorrhoids is comparable to PPH,but the operation is simpler and less invasive,which can shorten the length of hospital stay and effectively reduce the occurrence of postoperative complications.
