脓毒症患者外周血miR-98-5p与全身炎症反应激活及预后的相关性研究

摘　　要：目的研究脓毒症患者外周血miR-98-5p与全身炎症反应激活及预后的相关性。方法选择2019年1月至2021年1月期间自贡市第四人民医院收治的102例脓毒症患者作为脓毒症组,同期体检的100例健康志愿者作为对照组,采用荧光定量PCR法检测外周血miR-98-5p的表达水平,采用Elisa法检测血清炎症细胞因子TNF-α、HMGB-1、IL-1β的含量,采用急性生理和慢性健康评分Ⅱ(APACHEII)、序贯性器官衰竭评分(SOFA)评分评估脓毒症病情,采用28d生存情况评估预后。结果脓毒症组患者外周血miR-98-5p的表达水平低于对照组,差异有统计学意义(P<0.05);脓毒症组中miR-98-5p表达≥中位数患者的APACHEII、SOFA评分低于miR-98-5p表达<中位数患者,差异有统计学意义(P<0.05);脓毒症组中miR-98-5p表达≥中位数患者的血清TNF-α、HMGB-1、IL-1β含量低于miR-98-5p表达<中位数患者,差异有统计学意义(P<0.05);经K-M曲线分析,脓毒症组中miR-98-5p表达≥中位数患者的28 d累积生存率高于miR-98-5p表达<中位数患者差异有统计学意义(P<0.05);外周血miR-98-5p表达水平对脓毒症患者28 d生存具有预测价值(P<0.05)。结论脓毒症患者外周血miR-98-5p表达降低与病情加重、全身炎症反应激活及预后不良有关。Objective To study the correlation between miR⁃98⁃5p in peripheral blood with systemic inflammatory response activation and prognosis in patients with sepsis.Methods A total of 102 patients with sepsis admitted to the intensive care unit of this Hospital from January 2019 to January 2021 were selected as the sepsis group,and 100 healthy volunteers were selected as the control group.The expression level of miR⁃98⁃5p in peripheral blood was detected by fluorescence quantitative PCR.The contents of serum inflammatory cytokines TNF⁃α,HMGB⁃1 and IL⁃1βwere detected by ELISA.APACHEII and sequential organ failure score(SOFA)were used to evaluate the condition of sepsis,and 28⁃day survival was used to evaluate the prognosis.Result The expression level of miR⁃98⁃5p in the sepsis group was lower than that in the control group(P<0.05).The APACHEⅡ,SOFA score of patients with miR⁃98⁃5p expression≥median in the sepsis group were lower than those with miR⁃98⁃5p expression<median(P<0.05).The serum TNF⁃α,HMGB⁃1,IL⁃1βcontents of patients with miR⁃98⁃5p expression≥median in the sepsis group were lower than those with miR⁃98⁃5p expression<median(P<0.05).K⁃M curve analysis showed that the 28⁃day cumulative survival rate of patients with miR⁃98⁃5p expression≥median in the sepsis group were higher than those with miR⁃98⁃5p expression<median(P<0.05).The expression level of miR⁃98⁃5p in peripheral blood has predictive value for the 28⁃day survival of sepsis patients.Conclusion The decreased expression of miR⁃98⁃5p in peripheral blood of patients with sepsis is associated with aggravation of disease,activation of systemic inflammatory response and poor prognosis.
