益肾清浊汤对慢性肾脏病肝肾阴虚型患者尿蛋白及氧化应激指标的影响

摘　　要：目的评估益肾清浊汤对2-3期慢性肾脏病肝肾阴虚型患者尿蛋白定量及氧化应激指标的影响,并对该方可能的作用机制加以探讨。方法某两家医院2~3期慢性肾脏病肝肾阴虚型患者94例,随机分成治疗组和对照组各47例,对照组进行常规治疗,治疗组在常规治疗基础上加用益肾清浊汤的口服治疗,以8周作为1个疗程。检测两组治疗前后的24 h尿蛋白定量水平、血清丙二醛(MDA)、谷胱甘肽过氧化物酶(GSH-Px)以及血清超氧化物歧化酶(SOD)含量,并观察不良反应。结果治疗前,两组24h尿蛋白定量、MDA、GSH-Px、SOD水平差异无统计学意义(P>0.05);治疗后,两组24h尿蛋白定量水平、MDA含量下降,GSH-Px、SOD升高,差异具有统计学意义(P<0.05),与对照组相比,治疗组更为明显(P<0.05)。两组均未出现不良反应。结论益肾清浊汤联合西药治疗能减少2〜3期慢性肾脏病肝肾阴虚型患者的尿蛋白排泄,且安全性良好,可能与抑制氧化应激反应有关。OBJECTIVE To evaluate the clinical therapeutic effect of YiShen QingZhuo Decoction on patients with Yin Deficiency of Liver and Kidney of stage 2-3 chronic kidney disease,and to explore the possible mechanism of the prescription.METHODS 94 patients were divided into a treatment group and a control group randomly,47 cases in each group.The control group received conventional treatment for 8 weeks,and the treatment group were reated with YiShen QingZhuo Decoction on the basis of the conventional treatment for 8 weeks.Detect the quantitative levels of 24h urine protein before and after treatment,detect the levels of serum malondialdehyde(MDA),glutathione peroxidase(GSH-Px)and serum superoxide dismutase(SOD)before and after treatment in the two groups,and observe adverse reactions.RESULTS After treatment,the quantitative levels of 24h urine protein and MDA in the two groups were reduced(P<0.05),GSH-Px and SOD were increased(P<0.05),especially in the treatment group(P<0.05).There were no adverse reactions in the two groups.CONCLUSION YiShen QingZhuo Decoction can sig-nificandy reduce the level of urine protein in patients with good safety,which may be related to the inhibition of oxidative stress.
