髂骨骨破坏诊断Rosai-Dorfman病一例

摘　　要：报道1例软组织Rosai-Dorfman病累及髂骨病例。患者为55岁女性,主诉左髋部疼痛1月余,骨盆X线检查发现左髂骨体骨质破坏。查体:左髋外观正常,无红肿,外侧局部压痛(+),左髋活动略受限,左足背动脉搏动正常,皮肤感觉正常。患者盆腔肿块切除术治疗,术后病理检查:以泡沫样组织细胞增生为主的病变伴多量淋巴细胞、浆细胞、中性粒细胞等炎细胞浸润及局部多核巨细胞反应,可见胆固醇结晶,结合免疫组化S-100(+),诊断考虑为Rosai-Dorfman病。To report a case of soft tissue Rosai-Dorfman involving the ilium.This case is a 55-year-old woman complaining of left hip pain for more than 1 month.Pelvis X-ray revealed bony destruction of the left ilium body.Physical examination showed normal appearance of the left hip without redness or swelling.The tenderness test of the lateral hip was positive and the movement of the left hip was lightly limited.Both the dorsal foot artery pulsation and the sensation of the skin of the left foot were normal.The patient underwent to remove the lump in the ilium.The postoperative pathological examination showed hyperplasia of the foamy tissue cells,and infiltration of the inflammatory cells such as lymphocytes,plasma cells,and neutrophils.Multinucleated giant cell reaction,as well as the cholesterol crystals,could been seen.Combined with the immunohistochemical examination of S-100(+),this patient could be diagnosed as Rosai-Dorfman.
