摘 要:目的:了解桥本甲状腺炎(HT)患者血清中25-羟基维生素D[25-(OH)Vit D]水平情况,并探讨其与甲状腺激素和自身抗体的相关性。方法:选取2018年1月-2021年2月于深圳市第二人民医院就诊并确诊的107例HT患者为HT组,同时选择80例体检健康者为对照组。分别检测25-(OH)VitD、游离三碘甲状腺原氨酸(FT_(3))、游离甲状腺素(FT_(4))、促甲状腺激素(TSH)、甲状腺球蛋白抗体(TG-Ab)及甲状腺过氧化物酶抗体(TPO-Ab)水平,并对结果进行统计分析。结果:HT组25-(OH)Vit D、FT_(3)及FT_(4)水平分别为(45.92±8.17)nmol/L、(3.91±0.72)pmol/L及(12.75±1.61)pmol/L,均低于对照组的(78.34±9.26)nmol/L、(5.28±0.66)pmol/L及(18.94±2.30)pmol/L,而HT组TSH、TPO-Ab及TG-Ab水平分别为(5.76±1.49)mIU/ml、(412.60±103.87)IU/ml及(481.25±152.64)IU/ml,均高于对照组的(1.83±0.61)mIU/ml、(20.25±6.43)IU/ml及(13.86±3.71)IU/ml,差异均有统计学意义(P<0.05)。甲功正常组25-(OH)Vit D、FT_(3)及FT_(4)水平均高于亚甲减组和甲减组,且亚甲减组均高于甲减组,甲功正常组TSH、TPO-Ab及TG-Ab水平均低于亚甲减组和甲减组,且亚甲减组均低于甲减组,差异均有统计学意义(P<0.05)。经Pearson相关性分析,HT组25-(OH)Vit D与FT_(3)、FT_(4)水平呈正相关,而与TSH、TPO-Ab及TG-Ab水平呈负相关(P<0.05)。结论:HT患者血清中25-(OH)Vit D水平明显降低,且与甲状腺激素和自身抗体水平有一定相关性。维生素D水平降低可能在HT发生和发展中发挥了一定的作用。Objective:To investigate the level of 25-hydroxyvitamin D[25-(OH)Vit D]in serum of patients with Hashimoto thyroiditis(HT),and to explore the correlation between it and thyroid hormones and autoantibodies.Method:A total of 107 patients diagnosed with HT in the Second People’s Hospital of Shenzhen from January 2018 to February 2021 were selected as the HT group,and 80 healthy subjects were selected as the control group.The levels of 25-(OH)VitD,free triiodothyronine(FT_(3)),free thyroxine(FT_(4)),thyroid stimulating hormone(TSH),thyroid globulin antibody(TG-Ab)and thyroid peroxidase antibody(TPO-Ab)were detected,and the results were analyzed statistically.Result:The levels of 25-(OH)Vit D,FT_(3)and FT_(4)in the HT group were(45.92±8.17)nmol/L,(3.91±0.72)pmol/L and(12.75±1.61)pmol/L,respectively,which were lower than(78.34±9.26)nmol/L,(5.28±0.66)pmol/L and(18.94±2.30)pmol/L in the control group,while the levels of TSH,TPO-Ab and TG-Ab in the HT group were(5.76±1.49)mIU/ml,(412.60±103.87)IU/ml and(481.25±152.64)IU/ml,respectively,which were higher than(1.83±0.61)mIU/ml,(20.25±6.43)IU/ml and(13.86±3.71)IU/ml in the control group,the differences were statistically significant(P<0.05).The levels of 25-(OH)Vit D,FT_(3)and FT_(4)in the normal thyroid function group were higher than those in the subhypothyroidism group and hypothyroidism group,and those in the subhypothyroidism group were higher than those in the hypothyroidism group;the levels of TSH,TPO-Ab and TG-AB in normal thyroid function group were lower than those in subhypothyroidism group and hypothyroidism group,and those in subhypothyroidism group were lower than those in hypothyroidism group,the differences were statistically significant(P<0.05).Pearson correlation analysis showed that 25-(OH)Vit D in the HT group were positively correlated with the levels of FT_(3)and FT_(4),but negatively correlated with the levels of TSH,TPO-AB and TG-Ab(P<0.05).Conclusion:The level of 25-(OH)Vit D in HT patients is significantly decreased,and it is correlate
