摘  要:目的:探究全身亚低温治疗新生儿缺氧缺血性脑病(hypoxic ischemic encephalopathy,HIE)的效果。方法:选取泉州市儿童医院收治的新生儿缺氧缺血性脑病患者96例为本次研究对象,纳入时间为2018年1月-2020年12月。将其随机分为参照组、研究组,每组48例。参照组开展常规对症治疗,研究组在常规对症治疗基础上开展全身亚低温治疗。对比两组患儿的振幅整合脑电图、神经行为、并发症、发育状况。结果:研究组患儿振幅整合脑电图最低电压、最高电压均高于参照组,差异有统计学意义(P<0.05);研究组行为能力、原始反射、一般评价、被动肌张力等神经行为评分高于参照组,差异有统计学意义(P<0.05);研究组并发症发生率为4.17%,低于参照组的20.83%,差异有统计学意义(P<0.05);研究组大运动行为、精细运动行为、个人-社交行为、语言行为、社会适应等发育指标评分高于参照组,差异有统计学意义(P<0.05)。结论:采取全身亚低温治疗可改善新生儿缺氧缺血性脑病神经行为、振幅整合脑电图、发育情况,并减少并发症的发生。Objective:To explore the effect of systemic mild hypothermia on neonatal hypoxic ischemic encephalopathy (HIE).Method:A total of 96 patients with neonatal hypoxic ischemic encephalopathy treated in Quanzhou Children’s Hospital were selected as the research object,and the inclusion time was from January 2018 to December 2020.They were randomly divided into the reference group and the study group,with 48 cases in each group.The reference group carried out routine symptomatic treatment,and the study group carried out systemic mild hypothermia treatment on the basis of routine symptomatic treatment.The amplitude integrated electroencephalograhpy (EEG),neurobehavior,complications and development of the two groups were compared.Result:The lowest and highest voltage of amplitude integrated EEG in the study group were higher than those in the reference group,the differences were statistically significant (P<0.05);the neurobehavioral scores of behavioral ability,primitive reflex,general evaluation and passive muscle tension in the study group were higher than those in the reference group,and the differences were statistically significant (P<0.05);the incidence of complications in the study group was 4.17%,lower than 20.83% in the reference group,and the difference was statistically significant (P<0.05);the scores of development indexes such as large motor behavior,fine motor behavior,personal social behavior,language behavior and social adaptation in the study group were significantly higher than those in the reference group,the differences were statistically significant (P<0.05).Conclusion:Systemic mild hypothermia can improve the neurobehavior,amplitude integrated EEG and development of neonatal with hypoxic ischemic encephalopathy,and reduce the incidence of complications.
