NUSS手术前后漏斗胸患儿肺功能改善疗效分析

摘　　要：目的以昆明市延安医院胸外科收治的漏斗胸患儿为研究对象,探讨Nuss手术对患儿术前、术后肺功能的改善情况,评价该手术对漏斗胸患儿肺功能改善的疗效。方法收集2010年1月-2020年11月,昆明市延安医院胸外科实施Nuss手术的6-14岁患者90例。收集术前、术后3个月、术后6个月、术后1年四阶段肺功能检测资料,对资料结果采用SPSS17.0统计分析。结果 MEF50、MVV、BR于Nuss手术前后改变不明显。而MV、RV、VCMAX、FVC、FEV1术后3个月明显降低,术后6个月~1年逐渐恢复并超过术前。PEF、MEF25、MEF75、MEF75/25术后有所降低,6个月~1年逐渐恢复至术前水平。结论就漏斗胸患儿而言,术后短期3~6个月内肺功能改善不明显,而随着时间延长,患儿肺功能明显改善,远期效果明显。Objective In this study,it takes children with funnel chest in department of thoracic surgery,Yan ’an Hospital of Kunming,as research objects,discusses pulmonary function improvement before and after the Nuss,and evaluates the effects of Nuss surgery on the preoperative and postoperative pulmonary function improvement of children with funnel chest. Methods 90 patients aged from 6 to 14 years old who underwent the Nuss in the Department of thoracic surgery,Kunming Yan’an hospital,from Jan. 2010 to Nov. 2020 were collected. Four stage pulmonary function test data of before operation,3 months after operation,6 months after operation,and 1 year after operation were collected,and the data results were analyzed by SPSS17. 0 statistics. Results MEF50,MVV,Br did not changed significantly before and after Nuss surgery. However, MV, RV, VCmax, FVC, FEV1 significantly decreased in 3 months after surgery and gradually recovered from 6 months to 1 year after surgery and exceeded those before surgery.The levels of PEF,MEF25,MEF75,and MEF75/25 decreased after surgery and gradually recovered to their preoperative levels at 6 months to 1 year. Conclusions As far as children with pectus excavatum,lung function does not improve significantly in the short term within 3-6 months after surgery,whereas it improves significantly over time,and the long-term outcome is obvious.
