MSCT灌注成像对肝纤维化、肝硬化的鉴别诊断价值分析

摘　　要：目的分析MSCT灌注成像对肝纤维化、肝硬化的鉴别诊断价值。方法回顾分析本院2017年3月至2019年7月收治的肝纤维化和肝硬化患者的临床资料,分别为89例、40例,所有患者均经手术或病理检查确诊。对患者所得图像进行分析,比较不同分期肝纤维化与肝硬化灌注参数值(HAP、PVP、TLP、HBF、HPI、TTP、BV)。结果S_(1)-S_(2)期、S_(3)-S_(4)期肝纤维化患者HAP、PVP、TLP、HBF、HPI、TTP、BV值与肝硬化患者比较差异有统计学意义(P<0.05),肝硬化PVP、TLP、HBF、BV均低于S_(1)-S_(2)期、S_(3)-S_(4)期肝纤维化患者;S_(3)-S_(4)期肝纤维化患者HAP值为29.11±2.35,S_(1)-S_(2)期肝纤维化患者HAP值为28.12±3.55,两者比较差异无统计学意义(P>0.05)。结论MSCT灌注成像对肝纤维化、肝硬化的鉴别诊断有较高的应用价值,可为两者鉴别提供参考依据,值得临床推广。Objective To analyze the differential diagnosis value of MSCT perfusion imaging for liver fibrosis and cirrhosis.Methods The clinical data of 89 patients with liver fibrosis and 40 patients with cirrhosis admitted from March 2017 to July 2019 in our hospital were retrospectively analyzed.All patients were diagnosed by surgery or pathology.The images of the patients were analyzed to compare the perfusion parameters(HAP,PVP,TLP,HBF,HPI,TTP,BV)of different stages of liver fibrosis and cirrhosis.Results The differences in HAP,PVP,TLP,HBF,HPI,TTP,and BV between patients with liver fibrosis in S_(1)-S_(2) and S_(3)-S_(4) and the patients with cirrhosis were statistically significant(P<0.05).The PVP,TLP,HBV,and BV of patients with liver cirrhosis were lower than those of patients with liver fibrosis in S_(1)-S_(2) and S_(3)-S_(4).The HAP value of patients with liver fibrosis in S_(3)-S_(4) was 29.11±2.35,and the HAP value of patients with liver fibrosis in S_(1)-S_(2) was 28.12±3.55.There was no significant difference between the them(P>0.05).Conclusion MSCT perfusion imaging has high application value in the differential diagnosis of liver fibrosis and cirrhosis,which can provide a reference for the identification of them,and it is worthy of clinical promotion.
